研究目的：

主要目的：评估与传统Hartel前入路相比，改良Hartel入路半月神经节射频热凝术在三叉神经痛治疗中的精准性、有效性及安全性。  

Objectives of Study：

Main purpose: To evaluate the accuracy, effectiveness, and safety of the modified Hartel approach and semilunar ganglion radiofrequency thermocoagulation in the treatment of trigeminal neuralgia compared with the traditional Hartel anterior approach.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合国际头面痛学会分类委员会确定的原发性三叉神经痛的诊断标准；疼痛部位为三叉神经第三支分配区域
2.病人均曾服用卡马西平，但后续发生疼痛不能控制或难以忍受药物的不良反应；
3.年龄55-75岁；
4.病人术前均行颅内MR检查，排除颅内占位病变所致的继发性三叉神经痛；
5.愿意接受经皮卵圆孔半月神经节射频热凝术，签署知情同意书。

Inclusion criteria

1. Meet the diagnostic criteria of primary trigeminal neuralgia determined by the Classification Committee of the International Society of Head and Facial Pain; The area of pain is the third branch of the trigeminal nerve
2. All the patients had taken carbamazepine, but the subsequent pain could not be controlled or the adverse reactions of the drugs were intolerable;
3. Aged 55 to 75 years;
4. Preoperative intracranial MR examination was performed for all patients, and secondary trigeminal neuralgia caused by intracranial space-occupying lesions was excluded.
5. Be willing to accept percutaneous foramen ovale radiofrequency thermocoagulation of semilunar ganglion, and sign the informed consent.

排除标准：

1.未控制的严重高血压、心脏病等疾病，不能耐受手术；
2.半年内罹患脑卒中患者；
3.凝血功能障碍；
4.穿刺部位感染；
5.精神障碍患者；
6.严重心理问题患者；
7.不能客观描述症状及术中不能配合的患者；
8.同时伴有其他部位疼痛患者；
9.面部畸形、假牙等原因导致口角外面部凹陷等异常。

Exclusion criteria：

1. Uncontrolled severe hypertension, heart disease and other diseases, cannot tolerate surgery;
2. Patients suffering from stroke within six months;
3. Blood coagulation dysfunction;
4. Infection at the puncture site;
5. Patients with mental disorders;
6. Patients with severe psychological problems;
7. Patients who cannot objectively describe symptoms and cannot cooperate during surgery;
8. Patients with pain in other parts at the same time;
9. Facial deformities, dentures and other reasons cause abnormalities such as depressions in the outer corners of the mouth.
