研究目的：

1.利用前瞻性、单中心、队列研究探究重复经颅磁刺激(repetitive Transcranial MagneticStimulation rTMS )对卒中后失眠患者的治疗效果； 2.采用阿森斯失眠量表（Athens Insomnia Scale,AIS）评估疗效的基础上，运用睡眠体征监测仪客观反映患者的睡眠情况，同时观察治疗前后细胞因子５-羟色胺（5-HT）的变化，从细胞因子水平上探讨其作用机制。  

Objectives of Study：

1. To explore the therapeutic effect of repetitive transcranial magnetic stimulation (rTMS) on post-stroke insomnia patients using a prospective, single-center, cohort study; 2. On the basis of evaluating the curative effect with the Athens Insomnia Scale (AIS), the sleep sign monitor was used to objectively reflect the patient's sleep status. At the same time, the changes of cytokine 5-hydroxytryptamine (5-HT) before and after treatment were observed, and its mechanism of action was discussed from the level of cytokines.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合第4次全国脑血管病学术会议修订的脑卒中诊断标准并经CT或MRI确诊的脑卒中(出血性脑卒中或缺血性脑卒中)患者；
2.参照《中国失眠症诊断与治疗指南2017版》制定的失眠诊断标准:
（1）睡眠潜伏期延长;入睡时间超过30min；
（2）睡眠维持障碍;夜间觉醒次数≥2次或凌晨早醒；
（3）睡眠质量下降;睡眠浅、多梦；
（4）总睡眠时间缩短，通常少于6h；
（5）日间残留效应;次晨感到头昏、精神不振、嗜睡、乏力等；
3.近1周内未使用镇静催眠药物；
4.本次脑卒中前无失眠现象；
5.对本研究知情同意并签署知情同意书。

Inclusion criteria

1. Patients with stroke (hemorrhagic stroke or ischemic stroke) who meet the stroke diagnostic criteria revised by the 4th National Cerebrovascular Disease Academic Conference and diagnosed by CT or MRI;
2. Refer to the diagnostic criteria for insomnia formulated in the ''Chinese Guidelines for the Diagnosis and Treatment of Insomnia 2017 Edition'':
(1) The sleep latency is prolonged; the time to fall asleep exceeds 30 minutes;
(2) Sleep maintenance disorder; the number of awakenings at night >= 2 times or early morning awakening;
(3) Decreased sleep quality; shallow sleep and more dreams;
(4) The total sleep time is shortened, usually less than 6h;
(5) Residual effect during the day; dizziness, lack of energy, lethargy, fatigue, etc. in the next morning;
3. No sedative and hypnotic drugs have been used in the past 1 week;
4. There was no insomnia before the stroke;
5. Informed consent to this study and signed the informed consent.

排除标准：

1.发生脑卒中前有精神、神经症状，不配合治疗者；
2.合并耳聋、失语症、严重认知功能障碍而无法正常交流者；
3.合并严重冠心病、呼吸系统疾病以及疼痛性疾病等导致失眠者；
4.经颅磁电刺激治疗禁忌证者，如颅内存在金属、心脏起搏器及颅内活动性出血等。

Exclusion criteria：

1. Those who have mental and neurological symptoms before stroke and do not cooperate with treatment;
2. Combined with deafness, aphasia, severe cognitive dysfunction and unable to communicate normally;
3. Patientswith severe coronary heart disease, respiratory system disease and pain disease that lead to insomnia;
4. Patients with contraindications to transcranial magnetic stimulation therapy, such as intracranial metal, pacemaker and active intracranial bleeding.
