研究目的：

探讨低级别滑车发育不良儿童复发性髌骨脱位术后髌骨形态的变化。  

Objectives of Study：

To investigate the changes in patellar morphology following soft tissue surgical correction of recurrent patellar dislocation in children with low-grade trochlear dysplasia.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)双侧复发性髌骨脱位(复发定义为一次以上的外伤性发作，导致髌骨在正常位置脱出);
(2)骨骺未闭合(骨骺闭合后骨形态无变化);
(3) Dejour A(浅层滑车> 150°)，由Dejour et al描述进行分组;
(4)髌股内侧韧带损伤。

Inclusion criteria

(1) bilateral recurrent patellar dislocation (recurrent being defined as more than one traumatic episode involving disruption of the normal position of the patella within the femoral groove);
(2) open growth plates (as no changes in bone morphology occur after physeal closure);
(3) Dejour A (shallow trochlea > 150°) as described by Dejour et al;
(4) medial patellofemoral ligament (MPFL) injury.

排除标准：

(1)骨骺闭合;
(2)沟角(SA) < 150°(>角150°提示儿童滑车发育不良);
(3)高度滑车发育不良(Dejour B, C, D) (当髌骨脱位伴有严重滑车发育不良时，简单的软组织平衡手术不能达到满意的效果，常常需要人工创建股沟);
(4)合并交叉韧带或副韧带损伤;
(5)大于ii级的类风湿关节炎或骨坏死伴软骨损伤。

Exclusion criteria：

(1) closed physes;
(2) a sulcus angle (SA) of < 150° (an angle of > 150° suggests trochlear dysplasia in children);
(3) a high-grade trochlear dysplasia (Dejour BC and D) (when patellar dislocation is accompanied by severe trochlear dysplasia, simple soft-tissue balancing procedures cannot achieve satisfactory results, and artificial creation of a femoral sulcus is often required);
(4) concomitant cruciate ligament or collateral ligament injury;
(5) rheumatoid arthritis or osteonecrosis with cartilage damage of greater than grade II.
