研究目的:

通过前瞻性观察患儿术中各时点DO2i水平,收集术后临床数据并比较分析,研究术中不同氧供指数范围术后急性肾损伤及其他不良结局的发生率,确定最佳术中DO2i调节范围,为优化婴幼儿先心病手术体外循环管理的目标导向灌注策略提供依据。  

Objectives of Study:

By prospectively observing the DO2i level of children at various time points during the operation, collecting postoperative clinical data and comparing and analyzing them, studying the incidence of postoperative acute kidney injury and other adverse outcomes with different oxygen supply index ranges during the operation, and determining the best intraoperative DO2i. The adjustment range provides a basis for optimizing the goal-oriented perfusion strategy for the management of extracorporeal circulation in infants and young children with congenital heart surgery.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1. 体重≤10kg的先心病患儿;
2. 择期行体外循环下心脏直视手术,且预计体外循环时间>60min;
3. 美国麻醉医师协会(ASA)分级: I-III级;
4. 完成知情同意书的签署。

Inclusion criteria

1. Children with congenital heart disease weighing ≤10kg;
2. Elective open-heart surgery under cardiopulmonary bypass, and the estimated time of cardiopulmonary bypass is greater than 60min;
3. American Association of Anesthesiologists (ASA) classification: I-III;
4. Complete the signing of the informed consent form.

排除标准:

1. 既往心脏手术史;
2. 合并肾功能不全(术前肌酐值>1.0mg/dl或需术前透析);
3. 新生儿(出生不满28天);
4. 术前七天内使用肾毒性药物:庆大霉素和万古霉素;
5. 术前需要体外生命支持;
6. 术前贫血(Hb<110g/L)或血红蛋白增高(Hb>150g/L);
7. 预计术中应用深低温停循环或选择性脑灌注;
8. 急诊手术;
9. 已参与其它临床研究项目;
10. 术前合并神经系统或肝等重要脏器异常。

Exclusion criteria:

1. Past cardiac surgery history;
2. With renal insufficiency (preoperative creatinine value>1.0mg/dl or preoperative dialysis required);
3. Newborns (under 28 days of birth);
4. Use nephrotoxic drugs within seven days before the surgery: gentamicin and vancomycin;
5. Need extracorporeal life support before surgery;
6. Preoperative anemia (Hb<110g/L) or increased hemoglobin (Hb>150g/L);
7. It is expected to apply deep hypothermic circulatory arrest or selective cerebral perfusion during the operation;
8. Emergency surgery;
9. Has participated in other clinical research projects;
10. Before the operation, the nervous system or liver and other important organs are abnormal.
