研究目的：

主要目的：探讨在重症监护病房(ICU)机械通气患者应用苯磺酸瑞马唑仑实施镇静治疗的有效性、安全性，以及对预后的影响。  

Objectives of Study：

Main purpose: This study intends to explore the effectiveness and safety of remimazolam besylate for sedation in mechanical ventilated patients in ICU.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥18周岁；
2.接受有创机械通气；
3.预计48小时内不能达到拔管条件；
4.需应用镇静药物，以保证舒适、安全并促进生命支持措施的实施；
5.受试者或其授权委托人知情同意并签署知情同意书。

Inclusion criteria

1. Age >=18 years old;
2. Accept invasive mechanical ventilation;
3. It is expected that the extubation condition cannot be reached within 48 hours;
4. Need to use sedative drugs to ensure comfort, safety and to promote the implementation of life support measures;
5. Subjects or their authorized principals gave informed consent and signed the informed consent form.

排除标准：

1.妊娠或哺乳期；
2.需要深镇静来辅助特殊治疗如俯卧位、使用肌松剂的患者；
3.已在ICU中插管(不包括在手术室或运输中的插管时间)超过12小时；
4.经充分的液体复苏和血管加压治疗MAP压力<50 mmHg；
5.心率<55次/分钟，患者在没有功能起搏器的情况下正在接受β受体阻滞剂或高级房室传导阻滞治疗者除外；
6.证实或怀疑脊髓损伤或其他可能导致永久或长期虚弱的病理状态；
7.心功能不全，左室EF≤30%；
8.重度颅脑损伤，格拉斯哥评分（GCS）＜8分；
9.肝功能损伤（Child-Pugh C级别）；
10.肾功能不全（急性：AKI3级、慢性：CKD3期及以上）；
11.患者病情危重，预后差，试验开始前患者或授权委托人放弃治疗；
12.重症肌无力患者；
13.支气管哮喘患者；
14.对研究药物或成分过敏；
15.患者参加其他临床试验研究。

Exclusion criteria：

1. Pregnancy or lactation;
2. Patients who need deep sedation to assist special treatments such as prone position and muscle relaxants;
3. Has been intubated in the ICU (excluding intubation time in the operating room or transport) for more than 12 hours;
4. MAP pressure <50 mmHg after adequate fluid resuscitation and vasopressor therapy;
5. Heart rate < 55 beats/min, except for patients who are receiving beta-blockers or advanced atrioventricular block therapy without a functional pacemaker;
6. Confirmed or suspected spinal cord injury or other pathological conditions that may lead to permanent or long-term debilitation;
7. Cardiac insufficiency, left ventricular EF<=30%;
8. Severe craniocerebral injury, Glasgow score (GCS) < 8;
9. Liver function damage (Child-Pugh C level);
10. Renal insufficiency (acute: AKI grade 3, chronic: CKD stage 3 and above);
11. The patient is in critical condition and has a poor prognosis, and the patient or authorized client gave up treatment before the start of the trial;
12. Myasthenia gravis patients;
13. Patients with bronchial asthma;
14. Allergic to the research drug or ingredient;
15. The patient participates in other clinical trial studies.
