研究目的：

探讨老年慢性阻塞性肺疾病的肺康复策略内容，结合现有慢性病管理理论，设计基于共享决策及健康素养的肺康复干预策略。在实施中不断优化完善，最终探索出一种院外老年 慢性阻塞性肺疾病 慢性病管理的干预策略，以提高老年 慢性阻塞性肺疾病 慢性病管理的技术水平，解决老年慢性阻塞性肺疾病慢性病管理面临的难题。通过干预策略的实施，提高患者的肺康复知晓率、健康素养及参与比例，减少医疗资源的使用，减轻患者家庭及社会的经济负担，提高患者的生活质量和满意度，弥合健康信息“鸿沟”，实现社会公平。  

Objectives of Study：

Combined with the existing chronic disease management theory,the study aims to design a pulmonary rehabilitation intervention strategy based on shared decision making and health literacy by exploring the content of pulmonary rehabilitation strategies for elderly patients with chronic obstructive pulmonary disease(COPD).In order to improve the technical level of chronic obstructive pulmonary disease(COPD) mannagement and solve the problems faced by the management of COPD in the elderly, an intervention strategy for COPD management in the elderly out of hospital has been explored. Through the implementation of intervention strategies, the awareness rate,health literacy and participation ratio of patients in pulmonary rehabilation will be improved, the use of medical resources will be reduced,the economic burden of patients' families and society will be reduced, the quality of life and satisfaction of patients will be improved,the digital divide will be closed,and social equity will be realized.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）经市级医疗机构明确诊断为COPD，处于稳定期，肺功能检查FEV1/FVC＜70%和FEV1%小于80%；
（2）老年人，年龄大于等于60岁；
（3）小学及以上文化水平，阅读理解及书写能力正常；
（4）无沟通障碍，能正常应答，可独立或通过研究者帮助填写问卷；
（5）自己或同居家人有智能手机，并且能够运用智能手机进行有效的电话沟通和（或）使用微信）
（6）无精神、认知障碍；
（7）知情同意。

Inclusion criteria

1. The patient was definitely diagnosed as COPD by municipal medical institutions and was in stable phase. Pulmonary function examination showed that FEV1/FVC was less than 70% and FEV1% was less than 80%;
2. the elderly, aged 60 years or more;
3. The patient has a primary school or above education level with normal reading and writing ability;
4. patients with no communication obstacle and normal response ,and patients can fill in the questionnaire independently or with the help of researchers;
5. patients or their caregivers have smart phones, and can conduct effective communication using telephone and/or WeChat;
6. No mental or cognitive impairment;
7. Informed consent.

排除标准：

（1）并发肺源性心脏病、呼吸衰竭；
（2）伴有严重的心、肝、肾疾病；
（3）近一年有心肌梗死、心绞痛发作；
（4）高血压患者高于正常范围，经药物无法控制者；
（5）体质过度衰弱，不能耐受肌肉力量测试；
（6）有运动后晕厥病史。

Exclusion criteria：

(1) patients complicated with pulmonary heart disease and respiratory failure;
(2) patients accompanied by serious heart, liver and kidney diseases;
(3) patients with myocardial infarction and angina pectoris occurred in the past year;
(4) patients with hypertension whose blood pressure is higher than normal range, and cannot be controlled by drugs;
(5) patients with physical weakness, unable to tolerate muscle strength test;
(6) A history of syncope after exercise.
