研究目的：

1.主要目的：探究地奥司明是否能减轻全膝关节置换术后患者下肢肿胀； 2.次要目的：探究地奥司明在全膝关节置换术后疼痛控制及加速康复方面的效果。  

Objectives of Study：

1. Main purpose: To explore whether diosmin can reduce swelling of lower extremities in patients after total knee arthroplasty; 2. Secondary objective: To explore the effect of diosmin on pain control and accelerated recovery after total knee arthroplasty.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.因膝关节重度骨关节炎需行初次单侧TKA手术患者；
2.年龄为18~80岁；
3.术前美国麻醉学家学会（American Society of Anesthesiologists，ASA）分级（麻醉风险评估）为Ⅰ~Ⅲ级；
4.使用止血带，手术时间90min内，使用20-25mg氨甲环酸止血。

Inclusion criteria

1. Patients who need primary unilateral TKA surgery due to severe knee osteoarthritis;
2. Aged 18 to 80 years;
3. The preoperative American Society of Anesthesiologists (ASA) classification (anesthesia risk assessment) is grade I-III;
4. Use a tourniquet, within 90 minutes of the operation time, use 20-25mg tranexamic acid to stop bleeding.

排除标准：

1.心、肺等重要脏器功能衰竭患者；
2.肝、肾功能异常(ALT、AST>正常值1.5倍，血BUN>-8.3 mmoll，血Scr>115 umol/L)；
3.糖尿病患者且血糖水平控制不佳，经研究者判断有感染风险者；
4.合并意识障碍或精神疾病；
5.有神经肌肉功能障碍影响下肢功能；
6.凝血功能严重障碍患者；
7.合并严重内、外科疾病或体质弱，无法耐受手术；
8.严重不稳定且使用韧带保留的TKA无法治愈；
9.体内存在活动性感染灶(全身或局部有感染病变)；
10.整个临床研究期间计划生育、哺乳期及妊娠期妇女；
11.3个月之内参加过其他临床试验者；
12.研究者认为因其他原因不适宜参加本次临床试验者；
13.膝关节屈曲畸形≥30°或内外翻畸形≥30°；
14.对本研究中使用药物过敏者；
15.阿片类药物成瘾者；
16.语言障碍及不能完成疼痛数字分级法评分（Visual analogue scale，VAS）；
17.合并静脉系统疾病或静脉功能不全者；
18.BMI > 40 kg/m2 的患者。

Exclusion criteria：

1. Patients with heart, lung and other important organ failure;
2. Abnormal liver and kidney function (ALT, AST> 1.5 times the normal value, blood BUN> -8.3 mmol/L, blood Scr> 115 umol/L);
3. Diabetic patients with poor control of blood sugar levels, who are judged by the researchers to be at risk of infection;
4. Combined with disturbance of consciousness or mental illness;
5. Neuromuscular dysfunction affects lower extremity function;
6. Patients with severe coagulation disorders;
7. Combined with severe medical and surgical diseases or weak constitution, unable to tolerate surgery;
8. Severely unstable and incurable with ligament-preserving TKA;
9. There are active infection foci in the body (systemic or local infection lesions);
10. Women in family planning, lactation and pregnancy during the entire clinical research period;
11. Those who have participated in other clinical trials within 3 months;
12. The investigator believes that it is not suitable to participate in this clinical trial due to other reasons;
13. Knee flexion deformity >=30° or varus deformity >=30°;
14. Those who are allergic to the drugs used in this study;
15. Opioid addicts;
16. Language barriers and inability to complete the pain digital grading scale (Visual analogue scale, VAS);
17. Patients with venous system disease or venous insufficiency;
18. Patients with BMI > 40 kg/m2.
