初步评价自体脂肪脱细胞活性蛋白治疗膝关节骨关节炎的安全性及有效性  

Objectives of Study：

To determine the safety effectiveness of CEFFE in treatment of knee osteoarthritis

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1) 年龄20~80周岁（含边界值），男性或女性；
2) BMI在18.5~30kg/m2（含边界值）；
3) 依据《骨关节炎诊疗指南》（2018版）膝关节骨关节炎的诊断标准，近1个月内膝盖有反复的膝关节疼痛，并符合下列标准中的任意两项者：
? X线片（站立位或负重位）示关节间隙变窄、软骨下骨硬化和（或）囊性变、关节边缘骨赘形成；
? 年龄≥50岁；
? 晨僵时间≤30分钟；
? 活动时有骨摩擦音（感）；
4) 膝关节Kellgren&Lawrence分级I~III级；
5) 基线时，美国西部安大略和麦克玛斯特大学（Western Ontario and McMaster Universities, WOMAC）疼痛子量表评分为7~17分（含边界值）（5项，0~20分）（仅接受所有止痛药停用后至少48小时的WOMAC评分）；
6) 受试者同意在首次接受研究药物治疗的7天前停用所有治疗OA的止痛药物（若药物5个半衰期超7天，则需在5个半衰期前停药），除外对乙酰氨基酚；
7) 受试者通常能活动，而非长期卧床或坐轮椅，不用拐杖或手杖的帮助能行走50米以上；
8) 受试者在试验期间以及末次给药90天内无妊娠计划且自愿采取有效避孕措施；
9) 自愿签署并提供书面知情同意书。

Inclusion criteria

1) Aged between 20 and 80 (including boundary value), male or female;
2) BMI between 18.5 and 30kg/m2 (including boundary value);
3) According to the diagnostic criteria of knee osteoarthritis in the Guidelines for the Diagnosis and Treatment of Osteoarthritis (2018 edition), those who have recurrent knee pain in the past 1 month and meet any two of the following criteria:
? Radiographs (standing or weight-bearing) showing joint space narrowing, subchondral osteosclerosis and/or cystic changes, and osteophyte formation at joint margins;
? Age ≥50;
? Morning stiffness ≤30 minutes;
? Bone friction sounds during activities;
4) knee Kellgren&Lawrence grade I~III;
5) At baseline, the Western Ontario and McMaster Universities (WOMAC) pain Subscale scores ranged from 7 to 17 (including boundary values) (5 items, 0 to 20 points) (only WOMAC scores at least 48 hours after cessation of all painkillers);
6) Subjects agree to discontinue all analgesics for OA 7 days prior to the first study drug treatment (5 half-lives for drugs exceeding 7 days), with the exception of acetaminophen;
7) Subjects are usually mobile, not bedridden or wheelchair-bound, and can walk more than 50 meters without the help of crutches or canes;
8) Subjects had no pregnancy plan and voluntarily took effective contraceptive measures during the trial period and within 90 days of the last administration;
9) Voluntarily sign and provide written informed consent.

排除标准：

1) 经研究者判断，受试者存在外伤史，伤害影响到目标膝关节；
2) 筛选前6个月，有涉及目标膝关节的明显外伤；
3) 受试者存在膝关节以外的需治疗的同侧下肢关节OA、或患有其他影响关节的炎症性疾病（如：类风湿性关节炎、银屑病关节炎、痛风性关节炎、症状性软骨钙化、骨坏死、活动性感染），或根据研究者的判断，可能会影响目标膝关节疼痛和功能的评估的外周或中枢神经病变（如：背痛、髋关节疼痛、椎间盘突出、坐骨神经痛、纤维肌痛、糖尿病性神经病变等）；
4) 目标膝关节患有明显外翻/内翻畸形（>15°内外翻，以及明显屈曲挛缩畸形>5°）；
5) 计划在研究期间内对同侧膝关节以外任何下肢关节进行手术治疗；
6) 目标膝关节存在感染性关节炎或既往史；
7) 在筛选期目标膝关节有活动性关节感染或注射部位区域的皮肤破溃、感染或存在可能干扰关节腔内注射和/或注射部位评价的任何显著慢性皮肤病；
8) 受试者在筛选前30天内使用过阿片类药物、或糖皮质激素；或者研究期间需要使用阿片类药物或糖皮质激素进行长期治疗者；
9) 治疗开始前6个月内膝关节（双侧或任意一侧）腔内接受过任何药物注射治疗的患者；
10) 对侧膝骨关节炎，或任何髋骨关节炎症状，使用对乙酰氨基酚不能缓解/或需要接受任何方案终止治疗；
11) 在筛选前8周内使用过氨基葡萄糖，硫酸软骨素，双醋瑞因或鳄梨/大豆提取物者；
12) 受试者患有哮喘，需要全身性糖皮质激素治疗。但仅需吸入性糖皮质激素治疗的哮喘患者可入选；
13) 具有MRI检测禁忌症，包括但不限于：体内安装心脏起搏器、除颤器、心脏支架、人工心脏瓣膜、动脉瘤术后金属夹、植入体内的药物灌注装置、植入体内的任何电子装置（神经刺激器、骨骼生长刺激器）、血管内栓塞钢圈、滤器、心电记录监护器、金属缝合线、体内有弹片或铁砂粒者、骨折手术后固定钢板及钢钉、人工耳蜗、中耳移植物、眼内金属异物等；幽闭恐惧症、危重患者等；
14) 肝功能、肾功能异常：筛选时AST或ALT>2×正常值上限（Upper limits of normal, ULN）；或血肌酐> 1.5 × ULN，或肌酐清除率≤ 50 mL/min；
? 注：男性肌酐清除率（mL/min）＝［（140－年龄）×体重（kg）］/［0.818×Cr（umol/L）］；女性肌酐清除率（mL/min）=男性肌酐清除率（mL/min）×0.85；
15) 受试者因任何类型的活动性恶性肿瘤，正在接受全身化疗；或在筛选前5年内，被诊断出癌症（但不包括皮肤的鳞状细胞癌或基底细胞癌）；
16) 经研究者判断，在入组前，12导联心电图（Electrocardiogram, ECG）出现具有临床意义的异常；包括心率>100 bpm，或QRS >120 ms，或PR >220 ms；
17) 心脏基础疾病：在入组前6个月内，发生过下述心血管事件：心肌梗死或行冠状动脉搭桥手术或创伤性冠状动脉再通术或脑卒中；不稳定型心绞痛；变异性心绞痛；在休息或轻微运动后出现的症状性充血性心力衰竭（美国纽约心脏病协会心功能分级Ⅲ-Ⅳ级或射血分数≤35%）；
18) 未能有效控制的高血压（定义为经规范化降压药治疗后收缩压≥140 mmHg和/或舒张压≥90 mmHg）；
19) 乙肝表面抗原、丙肝抗体、艾滋病抗体、梅毒抗体检查阳性者；
20) 已知或怀疑对研究药物的活性或非活性成分过敏者；
21) 对对乙酰氨基酚过敏或有超敏反应史；
22) 有明确的精神障碍史，或有精神类药物滥用、吸毒史；
23) 酒精滥用（定义为每天饮酒>2单位/每周饮酒>14单位，饮酒1单位相当于360ml啤酒或45ml酒精含量40%的烈酒或150ml葡萄酒）或药物滥用者；
24) 妊娠期、哺乳期妇女以及未行绝育手术/未绝经/不愿意在研究期间使用医学认可的有效避孕措施的育龄期妇女；未行绝育手术/不愿意在研究期间使用医学认可的有效避孕措施的男性；
25) 在筛选前3个月内参加过其他任何临床试验者；
26) 研究者认为其他不适合参加本试验的情况。

Exclusion criteria：

1) According to the researcher's judgment, the subject has a history of trauma, and the injury affected the target knee joint;
2) 6 months prior to screening, there were obvious injuries involving the target knee joint;
3) Subjects have ipsilateral lower extremity joint OA that requires treatment other than kneejoint, or have other inflammatory diseases that affect joints (e.g. Rheumatoid arthritis, psoriatic arthritis, gouty arthritis, symptomatic cartilage calcification, osteonecrosis, active infection), or according to the researcher's judgment, may affect the target of knee pain and function evaluation of peripheral and central nervous pathological changes (such as back pain, hip pain, disc herniation, sciatica, fibromyalgia, diabetic neuropathy, etc.);
4) The target knee joint has significant valgus/varus deformity (>15° varus, and significant flexion contracture deformity >5°);
5) Surgical treatment of any lower limb joint other than the ipsilateral knee is planned during the study period;
6) The target knee joint has infectious arthritis or previous history;
7) During the screening period, the target knee joint has active joint infection or skin ulceration, infection or any significant chronic skin disease that may interfere with intra-articular injection and/or injection site evaluation;
8) Subjects have used opioids or glucocorticoids within 30 days prior to screening; Or requiring long-term treatment with opioids or glucocorticoids during the study period;
9) Patients who received any drug injection in the lumen of the knee (bilateral or either) within 6 months prior to the start of treatment;
10) Contralateral knee osteoarthritis, or any symptoms of hip osteoarthritis, which cannot be relieved by acetaminophen use/or require termination of any regimen;
11) Had used glucosamine, chondroitin sulfate, diacetyl or avocado/soybean extract within 8 weeks prior to screening;
12) Subjects had asthma and required systemic glucocorticoid therapy. However, patients with asthma treated only with inhaled corticosteroids were eligible.
13) Have MRI contraindications, including but not limited to: Body installed pacemakers and defibrillators, stents and artificial heart valve, aneurysm surgery metal clips, drug infusion device implanted in our bodies, any electronic devices implanted in our bodies (nerve stimulator, bone growth stimulator), endovascular embolization rims, filter, ECG record monitor, metal suture, the body with shrapnel or iron ore pellets, fracture fixed plate and after the operation Steel nail, cochlear implant, middle ear graft, metal foreign body in eye, etc. Claustrophobia, critically ill patients, etc.
14) Abnormal liver and kidney function: AST or ALT>2× Upper limits of normal (ULN) at screening time; Or serum creatinine > 1.5 × ULN, or creatinine clearance ≤ 50 mL/min;
? Note: Male creatinine clearance rate (mL/min) = [(140- age) × body weight (kg)] / [0.818×Cr (umol/L)]; Creatinine clearance rate of female (mL/min) = male (mL/min) ×0.85;
15) Subject is receiving systemic chemotherapy for any type of active malignancy; Or have been diagnosed with cancer (but not squamous cell carcinoma or basal cell carcinoma of the skin) within 5 years prior to screening;
16) Prior to enrollment, 12-lead Electrocardiogram (ECG) abnormalities were clinically significant, as determined by the investigator; Included heart rate >100 BPM, or QRS >120 ms or PR >220 ms;
17) Underlying heart disease: any of the following cardiovascular events occurred within 6 months prior to enrollment: myocardial infarction or coronary artery bypass surgery or traumatic coronary artery recanalization or stroke; Unstable angina pectoris; Variant angina pectoris; Symptomatic congestive heart failure (New York Heart Association Heart function grade ⅲ - ⅳ or ejection fraction ≤35%) after rest or mild exercise;
18) Hypertension that is not effectively controlled (defined as systolic blood pressure ≥140 mmHg and/or diastolic blood pressure ≥90 mmHg after standardized antihypertensive medication);
19) Hepatitis B surface antigen, hepatitis C antibody, AIDS antibody, syphilis antibody test positive;
20) Known or suspected allergy to the active or inactive ingredients of the drug under study;
21) Allergic to paracetamol or history of hypersensitivity reaction;
22) Have a clear history of mental disorder, or have a history of psychotropic drug abuse or drug addiction;
23) Alcohol abuse (defined as >2 units per day / >14 units per week, with 1 unit equivalent to 360ml beer or 45ml 40% spirits or 150ml wine) or drug abuse;
24) pregnant and lactating women and women of childbearing age who have not undergone sterilization/menopause/do not wish to use medically approved effective contraceptive methods during the study period; Men who have not undergone sterilization/do not wish to use medically approved effective contraception during the study period;
25) Have participated in any other clinical trials within 3 months prior to screening;
26) Other conditions that the investigator considers inappropriate for participation in the study.
