研究目的：

1.探讨基于家庭参与的探视模式对改善心脏术后谵妄的有效性； 2.评估在ICU开展基于家庭参与的探视模式的安全性。  

Objectives of Study：

1. To explore the effectiveness of visit mode based on family engagement in improving delirium after cardiac surgery; 2. To evaluate the safety of visit mode based on family engagement in ICU.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.心脏外科患者：
(1)年龄18~75周岁；
(2)接受心脏瓣膜手术且术后首次入住ICU；
(3)有直系亲属，可参与ICU探视；④取得患者或家属的知情同意。
2.患者家属：
(1)年龄≥18周岁；
(2)患者的父母、配偶、兄弟、姐妹或子女，承担照顾患者的主要责任，每天照顾时间最长且照顾时间超过2周；
(3)具备基本的交流沟通、阅读和理解能力。

Inclusion criteria

1. Cardiac surgery patients:
(1) Aged 18-75 years;
(2) Undergo heart valve replacement and be admitted to cardiacsurgery ICU for the first time;
(3) Have immediate relatives who can participate in intensive care unit visits;
(4) Obtain the informed consent of the patient or family members.
2. Family members of patients:
(1) Aged >= 18 years;
(2) Parents, spouses, brothers, sisters or children, who bear the main responsibility of taking care of the patients;
(3) Have the basic communication, reading and understanding ability.

排除标准：

1. 心脏外科患者：
(1)入院时存在谵妄；
(2)有神经系统疾病、器质性脑损伤或处于深度昏迷的患者；
(3)既往有精神疾病史和痴呆史；
(4)存在严重的视听功能障碍，无法进行沟通者；
(5)既往有酗酒史或药物滥用病史；
(6)急诊手术患者；
(7)参加其他相关研究；
(8)因病情需深度镇静患者，RASS评分≤ -3分。
2. 患者家属：有严重的躯体疾病、脑器质性疾病或精神疾病史。

Exclusion criteria：

1. Cardiac surgery patients:
(1) Delirium occurred at admission;
(2) Nervous system disease, organic brain injury, or deep coma;
(3) History of mental illness (including confirmed Alzheimer's disease);
(4) Previous cognitive or verbal communication disorders;
(5) History of alcoholism or drug abuse;
(6) Emergency surgery patients;
(7) Participation in other related research;
(8) Who need deep sedation, with RASS score <= - 3 points.
2. Family members of patients: with a history of serious physical diseases, brain organic diseases and mental diseases.
