研究目的:

龋病是口腔常见病、多发病,发病率高达30%-50以上。龋病未及时治疗,常导致牙齿发生不可复性牙髓炎或根尖周炎,目前临床不可复性牙髓炎和根尖周炎常规采用的方法是根管治疗术(RCT)。 虽然根管治疗的有效性是不可辩驳的,但是由于根管解剖复杂,根管内细菌难以去净;充填的惰性材料无法严密充填复杂的根管系统,也无法杀灭根管内残余细菌;根管惰性充填材料不能像天然牙髓组织一样为牙体组织提供神经营养支持,不能发挥牙髓组织的免疫防御作用。因此,根管治疗后的牙齿的牙本质更容易被细菌入侵;牙齿也会更容易因修复体边缘微渗漏再感染或导致根管内残存细菌扩增,这些是导致根管治疗或根管再治疗失败的主要原因之一,也是过去几十年来虽然根管治疗技术和设备改进明显,但是成功率仍无显著改变的原因。另外,牙髓组织丧失是导致根管治疗患牙牙齿冠折、根折甚至拔除的主要原因。如果能通过牙髓再生技术手段促进牙髓样组织长入根管,将带来以下优点:1、利用牙髓结缔组织的免疫防御功能杀灭细菌;2、防御外来细菌进一步入侵根管;3、显著减少牙根折裂的发生率。 牙髓再生术是一种手术方式,常规根管预备消毒后,利用自体血、血小板浓缩物或者自体干细胞募集趋化牙齿根尖孔外的干细胞进入根管,分化形成形成牙髓样组织。富血小板纤维蛋白衍生物的牙髓再生因为更富含纤维蛋白和生长因子、另外不需要根尖刺激出血,牙髓再生效果优于自体血牙髓再生效果;同时避免了干细胞体外培养扩增可能出现污染、干性丢失、基因突变等多种可能。浓缩生长因子(concentrated growth factor,CGF)是第三代血小板浓聚物,相比第二代富血小板纤维蛋白( PRF),富含更多三维网状结构的纤维蛋白基质及多种内源性生长因子。胶原作为一种天然支架材料,具有很好的生物相容和生物降解性能、在体内可以降解,免疫原性较低,是常用的天然组织工程材料,也是口腔临床常用的材料。研究发现,相比合成的可降解支架材料,胶原更适合牙髓再生。  

Objectives of Study:

The incidence of frequent oral diseases is as high as 30%-50 or more diseases not treated in a timely manner often lead to the occurrence of non-recurring dental myelitis or root typhus, the current clinical non-recurring dental myelitis and root typhus conventional method is root canal therapy (RCT). Although the effectiveness of root tube therapy is irrefutable, but because of the complex root tube anatomy, the bacteria in the root tube is difficult to clean, filling inert material can not be tightly filled with complex root tube system, can not kill residual bacteria in the root tube, root tube inert filling material can not provide neurotrophic support for dental tissue like natural pulp tissue, can not play the immune defense role of dental pulp tissue. As a result, the dental nature of teeth after root tube treatment is more susceptible to bacterial invasion, teeth are also more likely to be re-infected by micro-leakage of the edge of the prosthesis or lead to bacterial amplification of residual bacteria in the root tube, which is one of the main causes of root tube treatment or root tube retherapy failure, but also in the past few decades, although the root tube treatment technology and equipment improvements are obvious, but the success rate has not changed significantly. In addition, the loss of pulp tissue is the main cause of root tube treatment with tooth crown folding, root folding and even removal. If the pulp-like tissue can be promoted by means of pulp regeneration technology into the root tube, it will bring the following advantages: 1, the use of myelin connective tissue immune defense function to kill bacteria; Pulp regeneration is a surgical method, after the preparation of disinfection of conventional root tubes, the use of self-blood, platelet concentrate or stem cells to collect stem cells outside the root holes of the teeth into the root tube, differentiation to form a pulp-like tissue. The pulp regeneration of platelet fibroblast derivatives is better than that of the self-hematopoietic pulp regeneration because it is richer in fibrin and growth factors, and there is no need for root tip stimulation bleeding, while avoiding the possibility of contamination, dryness loss, gene mutation and other possibilities of stem cell in vitro culture amplification. Concentrated growth factor (concentration growth factor, CGF) is a third-generation platelet-rich polymer, rich in more three-dimensional mesh fibrin matrix and a variety of endogenous growth factors than the second generation of platelet fibrin (PRF). Collagen, as a natural stent material, has good biocompatibility and biodegradability, can be degraded in the body, immunogenicity is low, is commonly used natural tissue engineering materials, but also clinically commonly used materials in the oral cavity. The study found that collagen is more suitable for pulp regeneration than synthetic biodegradable stent materials.

纳入标准:

年龄在18至65岁之间,除第三磨牙的所有牙齿(牙根已发育完全),存在不可复性牙髓炎或根尖周炎症状,X线显示根尖周正常、根尖周膜腔增宽或根尖阴影。

Inclusion criteria

All teeth between the ages of 18 and 65 with the exception of the third grinding tooth (the root is fully developed) have symptoms of irreversible pulpitis or root tip circumliumitis, and X-rays show normal root circumcision, widening of the root tip circumcision cavity, or root tip shadow.

排除标准:

牙齿存在根折;牙齿无法直接树脂修复;牙齿大面积缺损导致以后难以检查牙髓活力;牙齿缺损至龈下2mm以上;牙周袋深度超过3mm;无对颌牙存在;无邻牙接触;存在系统性疾病;存在严重牙周疾病,牙根松动II度以上;内吸收;外吸收;根管严重钙化和髓腔闭锁。

Exclusion criteria:
