研究目的：

卒中急性期时，是脑水肿发生的高峰期，胃肠功能紊乱，若在此时给肠内营养治疗，容易导致呃逆，甚至是胃肠食物返流，引起误吸，从而增加肺部感染的可能性。据上我们提出可否早期干预患者胃肠功能障碍，使其得到改善，从而降低早期肠内营养的不耐受性；而针刺穴位可通过促进胃肠道蠕动功能，改善患者早期肠内营养耐受性；芒硝外敷可兼有抑制炎症反应、防治感染及促进胃肠道功能恢复等作用；二者联用希望达到改善重症脑卒中患者胃肠功能障碍。  

Objectives of Study：

The acute phase of stroke is the peak of cerebral edema and gastrointestinal dysfunction. If enteral nutrition treatment is given at this time, it is easy to cause hiccups, or even gastrointestinal food reflux, causing aspiration, thereby increasing the possibility of pulmonary infection. According to the above, we proposed whether it is possible to intervene early in patients with gastrointestinal dysfunction to improve it, so as to reduce the intolerability of early enteral nutrition. Acupoint acupuncture can improve patients' early intestinal nutrition tolerance by promoting gastrointestinal peristalsis function. External application of mirabilite can inhibit inflammation, prevent infection and promote gastrointestinal function recovery and other effects; The combination of the two is expected to improve gastrointestinal dysfunction in patients with severe stroke.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1)符合上述诊断标准；
2)发病72h内均经头部CT或MRI证实为急性脑梗塞、脑出血或混合性卒中；既往可有卒中史，但未残留明显神经功能缺损者；
3)美国国立卫生研究院卒中量表(National Institute of Health stroke scale, NIHSS)评分≥20分；
4)急性生理与慢性健康评分-Ⅱ(Acute Physiology and Chronic Health Evaluation, APACHE-Ⅱ)≥10分；
5)AGI分级为Ⅰ-Ⅱ级；
6)年龄18-80岁；
7)住院时间≥14天；
8)患者或家属知情并签署知情同意书。

Inclusion criteria

1) The above diagnostic criteria are met;
2) Acute cerebral infarction, cerebral hemorrhage or mixed stroke were confirmed by head CT or MRI within 72h after onset; The patient may have a history of stroke, but no significant residual neurological deficits;
3) National Institute of Health Stroke Scale (NIHSS) score ≥20;
4) Acute Physiology and Chronic Health Evaluation (APACHE- ⅱ)≥10;
5)AGI is graded as grade ⅰ - ⅱ;
6) Ages 18-80;
(7) Hospitalization ≥14 days;
8) The patient or family member is informed and signs the informed consent.

排除标准：

1)有消化道穿孔、肠梗阻、胃肠道术后及肠瘘等肠内营养禁忌证者；
2)肠道肿瘤患者；
3)妊娠或哺乳期妇女；
4)皮肤有感染、溃疡、瘢痕，不宜进行针刺穴位治疗者；
5)病情濒危，预计生存时间不足14d者；
6)血流动力学状态恶化，需增加血管活性药物剂量者；
7)年龄＜18岁或＞80岁；
8)急诊手术、颅脑外伤患者,接受溶栓治疗的患者；
9)家属拒绝进入本试验者。

Exclusion criteria：

1) Patients with contraindications of enteral nutrition such as gastrointestinal perforation, intestinal obstruction, gastrointestinal surgery and intestinal fistula;
2) Patients with intestinal tumor;
3) Pregnant or lactating women;
4) Patients with skin infection, ulcer or scar should not be treated by acupuncture at acupoints;
(5) those with an endangered condition and an estimated survival time of less than 14 days;
6) the hemodynamic state deteriorated and the dose of vasoactive drugs needed to be increased;
7) Age < 18 or > 80;
8) Patients undergoing emergency surgery, craniocerebral trauma, or thrombolytic therapy;
9) Family members refused to enter the experiment.
