研究目的：

为脑小血管病步态失稳患者的康复治疗探索新途径, 寻找更为优化和精准的康复方案，探讨大脑皮质运动神经功能重塑的机制，为降低跌倒风险，提高生活质量，减轻脑梗死运动功能障碍患者的个人及家庭压力，判断其病情及预后评价提供重要依据。  

Objectives of Study：

To explore a new way for the rehabilitation treatment of patients with gait instability of cerebrovascular disease, find a more optimized and accurate rehabilitation scheme, and explore the mechanism of motor nerve function remodeling of cerebral cortex, so as to provide an important basis for reducing the risk of falls, improving the quality of life, reducing the personal and family pressure of patients with motor dysfunction of cerebral infarction, judging their condition and evaluating their prognosis.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄45-65岁；②符合2015年中国脑小血管病诊治共识的诊断标准:头颅磁共振成像（magnetic resonance imaging，MRI）、 T2加权成像及液体衰减反转恢复序列（fluid attenuated inversion recovery，FLAIR）上脑白质对称性高信号，Fazekas评分≥2级和至少一个腔隙性梗死灶（非责任病灶）；③患者血压平稳（100-160/60-100mmHg）；④同意进行系统的认知和运动功能评价，能够理解治疗师或仪器指令，并能完成所要求的动作。⑤通过甘肃省人民医院伦理委员会批准。同意参加本研究，签署知情同意书。

Inclusion criteria

① Age 45-65 years; ② It meets the diagnostic criteria of the consensus on the diagnosis and treatment of small cerebrovascular diseases in China in 2015: cranial magnetic resonance imaging (MRI), T2 weighted imaging and symmetrical high signal in white matter on fluid attenuated inversion recovery (FLAIR), Fazekas score ≥ 2 and at least one lacunar infarct (irresponsible focus); ③ The patient's blood pressure was stable (100-160 / 60-100mmhg); ④ Agree to conduct systematic cognitive and motor function evaluation, be able to understand the instructions of the therapist or instrument, and be able to complete the required actions. ⑤ Approved by the ethics committee of Gansu Provincial People's hospital. Agree to participate in this study and sign the informed consent form.

排除标准：

①有视觉、理解功能障碍或其他原因，不能配合训练者；②其他原因的脑白质高信号，如多发性硬化、代谢或中毒性脑病、感染等；③可能的心源性栓塞；④其他原因引起的平衡及步态异常，如骨关节疾病，脊髓损伤，周围性神经病，感染性神经系统疾病等；⑤有严重的骨关节疾 病和关节肌肉疼痛剧烈，影响活动者。

Exclusion criteria：

① those who have visual and understanding dysfunction or other reasons and cannot cooperate with the training; ② High white matter signal from other causes, such as multiple sclerosis, metabolic or toxic encephalopathy, infection, etc; ③ Possible cardiogenic embolism; ④ Balance and gait abnormalities caused by other causes, such as bone and joint diseases, spinal cord injury, peripheral neuropathy, infectious nervous system diseases, etc; ⑤ There are serious bone and joint diseases and severe joint and muscle pain, affecting the active person.
