研究目的：

本研究拟探究积极监测是否适用于甲状腺低危潜在恶性隐匿性结节（≤1cm）以及探究可能影响甲状腺低危潜在恶性隐匿性结节随访的危险因素。此外，分析影响患者接受积极监测的因素以及积极监测相应的经济学效益。  

Objectives of Study：

This study aims to explore whether active monitoring is suitable for low-risk latent malignant thyroid nodules (≤ 1cm) and explore the risk factors that may affect the follow-up of low-risk latent malignant thyroid nodules. In addition, we analyzed the factors influencing patients to receive active monitoring and the corresponding economic benefits of active monitoring.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄≥18岁，性别不限；2.结节最大径≤1cm且依据Kwak等（2011年）版本TI-RADS≥4B级；3.无呼吸困难，声音嘶哑，局部胀痛等压迫症状；4.结节不靠近甲状腺被膜且无周围组织侵犯，如气管、食管、神经等；5.无淋巴结和远处转移证据； 6.无青少年或童年时期颈部放射暴露史；无甲状腺癌切除史及甲状腺癌家族史；7.愿意接受积极监测并且心理压力不大、能积极配合随访计划；8.理解疾病有进展可能，将来可能需要手术治疗；9.入组前患者详细了解试验性质、意义、可能的获益以及可能带来的不利及潜在风险情况下理解且自愿书面签署知情同意书。

Inclusion criteria

1. Age ≥ 18 years old, regardless of gender; 2. Maximum diameter of nodule ≤ 1cm and according to Kwak et al. (2011) version TI-RADs ≥ 4B; 3. No compression symptoms such as dyspnea, hoarseness, local pain; 4. Nodule not close to thyroid capsule and no surrounding tissue invasion, such as trachea, esophagus, nerve; 5. No evidence of lymph node and distant metastasis; 5; 6. No history of neck radiation exposure in adolescence or childhood; no history of thyroid cancer resection and family history of thyroid cancer; 7. Willing to accept active monitoring and low psychological pressure, and able to actively cooperate with the follow-up plan; 8. Understand that the disease may progress and may need surgical treatment in the future;
9. Before enrollment, the patients understood the nature, significance, possible benefits, possible disadvantages and potential risks of the trial in detail and signed the informed consent voluntarily.

排除标准：

1.结节最大径＞1cm或依据Kwak等（2011年）版本TI-RADS ＜4B 级；2.有临床症状；3.结节靠近甲状腺被膜或有周围组织侵犯，如气管、食管、神经等；4.有淋巴结或远处转移证据；5.有青少年或童年时期颈部放射暴露史；6.有甲状腺癌切除术或甲状腺癌家族史；7.心理压力大、无法积极配合随访计划；8.无法签署书面知情同意书。

Exclusion criteria：

1. The maximum diameter of the nodule> 1cm or TI-RADS <4B according to Kwak et al. (2011) version; 2. There are clinical symptoms; 3. The nodule is close to the thyroid capsule or has surrounding tissue invasion, such as trachea, esophagus, nerves, etc; 4. There is evidence of lymph node or distant metastasis; 5. There is a history of radiation exposure in the neck during adolescence or childhood; 6. There is a family history of thyroid cancer resection or thyroid cancer; 7. The psychological pressure is high, and the follow-up plan cannot be actively cooperated; 8 . Unable to sign written informed consent.
