研究目的：

肥胖症是世界性的健康问题。减重手术能快速持续减重、改善代谢紊乱，但其对术后的神经心理、认知、睡眠等的影响尚缺乏研究。报道显示，“肠道菌群-肠-脑轴”的变化可能与减重手术后肥胖和心理认知障碍的变化有关，然因果关系和机制并不明了。本研究拟采用宏基因组测序、神经影像学、神经心理量表评测、睡眠监测和生物标记物检测等手段，对手术干预及非手术干预的两个肥胖症队列进行定期的认知心理、睡眠、肠道菌群及代谢物检测，探索减重手术对心理认知功能、“肠道菌群-肠-脑轴”变化、代谢、睡眠及脑结构和功能的影响，揭示“肠道菌群-肠-脑轴”调控肥胖和神经心理、认知的机制，为肥胖症治疗提供新依据、新靶点。  

Objectives of Study：

Obesity is a worldwide health problem. Bariatric surgery can lose weight quickly and continuously and improve metabolic disorders, but its effects on the neuropsychology, cognition, and sleep after surgery are still lacking in research. Reports show that the changes in brain–gut–microbiome axis may be related to changes in obesity and mental cognitive impairment after bariatric surgery, but the causal relationship and mechanism are not clear. Our study intends to use metagenomic sequencing，neuroimaging, neuropsychological scale evaluation, sleep monitoring, and biomarker detection methods to conduct regular cognitive psychology, sleep, gut microbiome and metabolomics tests on the two obesity cohorts of surgical intervention and non-surgical intervention. The aim of this study is to explore the effects of bariatric surgery on mental and cognitive function, brain–gut–microbiome axis, metabolism, sleep, and brain structure and function and reveal the mechanism how the brain–gut–microbiome axis regulating obesity, neuropsychology and cognition, and provide new evidence and new targets for the treatment of obesity.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）符合2016年AACE/ACE肥胖指南中肥胖症诊断标准：BMI≥30kg/m2，男性腰围≥85cm，女性腰围≥74cm；（2）年龄18-55岁；（3）右利手（4）愿意进行神经心理学测试、神经影像、多导睡眠监测等检查，并签署知情同意书；（5）部分患者符合2019版中国肥胖及2型糖尿病外科治疗指南中手术适应症，无禁忌症，且愿意接受腹腔镜胃袖状切除术

Inclusion criteria

1）Meet the obesity diagnostic criteria in the 2016 AACE/ACE Obesity Guidelines: BMI≥30kg/m2, male waist circumference ≥85cm, female waist circumference ≥74cm;(2) Age 18-55 years old;(3) Right-handed;(4) Willing to conduct neuropsychological testing, neuroimaging, polysomnography examination, and signed informed consent form;（5) Some patients meet the surgical indications in 2019 edition of guidelines for surgical treatment of obesity and type 2 diabetes in China, have no contraindications, and are willing to carry out laparoscopic sleeve gastrectomy

排除标准：

1）既往或当前患神经系统疾病或严重的精神疾病；（2）明确的继发性和药物性肥胖；（3）妊娠或哺乳期妇女；（4）不能配合检查者；（5）既往胃肠道大手术史；（6）在研究前三个月或在研究中的任何时候使用抗生素，益生菌或益生元进行治疗（术前预防除外）；（7）严重原发性疾病；（8）发生严重并发症或产生安全性问题者；受试者中途提出中止要求及因其他原因而不愿意继续进行者。

Exclusion criteria：

1）Current or past neurological disease or serious mental illnessa; 2）Definitely diagnosed secondary obesity and drug-induced obesity; 3）Pregnant or breastfeeding women; 4) Unable to cooperate with examination;5)prior history of major gastrointestinal surgery; 6）Use antibiotics, probiotics or prebiotics for treatment in the three months before the study or at any time during the study (except for preoperative prophylaxis);7）Severe primary disease;8）Those who produce serious complications or safety problems; those who request to stop during the study or who are unwilling to continue due to other reasons.
