研究目的：

1. 主要目的：探究地奥司明是否能减轻全膝关节置换术后患者下肢肿胀 2. 次要目的：探究地奥司明在全膝关节置换术后疼痛控制及加速康复方面的效果  

Objectives of Study：

1. Main outcome: Explore whether diosmin can reduce the swelling of the lower limbs of patients after total knee arthroplasty 2. Secondary outcome: Explore the effect of diosmin on pain control and speed up recovery after total knee arthroplasty

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）因膝关节重度骨关节炎需行初次单侧TKA手术患者；
(2)年龄为18~80岁；
（3）术前美国麻醉学家学会（American Society of Anesthesiologists，ASA）分级（麻醉风险评估）为Ⅰ~Ⅲ级；
（4）使用止血带，手术时间90min内，使用20-25mg氨甲环酸止血。

Inclusion criteria

(1) Patients undergo unilateral TKA surgery due to severe osteoarthritis of the knee joint;
(2) 18~80 years old;
(3) American Society of Anesthesiologists（ASA） classification (anaesthesia risk assessment) is Ⅰ~Ⅲ
（4）Use tourniquet, use 20-25mg tranexamic acid to stop bleeding within 90 minutes of operation

排除标准：

（1）心、肺等重要脏器功能衰竭患者；
（2）肝、肾功能异常(ALT、AST>正常值1.5倍，血BUN>-8.3 mmoll，血Scr>115 umol/L)；
（3）糖尿病患者且血糖水平控制不佳，经研究者判断有感染风险者；
（4）合并意识障碍或精神疾病；
（5）有过膝关节手术史；
（6）有神经肌肉功能障碍影响下肢功能；
（7）凝血功能严重障碍患者；
（8）合并严重内、外科疾病或体质弱，无法耐受手术；
（9）严重不稳定且使用韧带保留的TKA无法治愈；
（10）体内存在活动性感染灶(全身或局部有感染病变)；
（11）整个临床研究期间计划生育、哺乳期及妊娠期妇女；
（12）3个月之内参加过其他临床试验者；
（13）研究者认为因其他原因不适宜参加本次临床试验者；
（14）膝关节屈曲畸形≥30°或内外翻畸形≥30°；
（15）对本研究中使用药物过敏者；
（16）阿片类药物成瘾者；
（17）语言障碍及不能完成疼痛数字分级法评分（Visual analogue scale，VAS）；
（18）合并静脉系统疾病或静脉功能不全者；
（19）BMI > 40 kg/m2 的患者

Exclusion criteria：

(1) Patients with failure of important organs such as heart and lungs;
(2) Abnormal liver and kidney function (ALT, AST>1.5 times the normal value, blood BUN>-8.3 mmoll, blood Scr>115 umol/L);
(3) Diabetic patients with poor blood glucose level control, who are at risk of infection as judged by the investigator;
(4) Concomitant disturbance of consciousness or mental illness;
(5) Have a history of knee surgery;
(6) Neuromuscular dysfunction affects lower limb function;
(7) Patients with severe coagulation disorders;
(8) Combined with severe internal or surgical diseases or weak physique, unable to tolerate surgery;
(9) TKA that is severely unstable and cannot be cured with ligament retention;
(10) There are active infection foci in the body (infectious lesions all over the body or part);
(11) Family planning, lactation and pregnancy women during the entire clinical study period;
(12) Those who have participated in other clinical trials within 3 months;
(13) The investigator believes that it is not suitable to participate in the clinical trial due to other reasons;
(14) Knee joint flexion deformity ≥30° or varus deformity ≥30°;
(15) Those who are allergic to the drugs used in this study;
(16) Opioid addicts;
(17) Language barriers and inability to complete the pain numerical scale (Visual analogue scale, VAS);
(18) Patients with venous system disease or venous insufficiency;
(19) Patients with BMI> 40 kg/m2
