评估上腔静脉节段性电隔离对于心房颤动患者射频消融治疗的有效性及安全性。  

Objectives of Study：

To assess the safety and efficacy of segmental isolation of superior vena cava for radiofrequency ablation in patients with atrial fibrillation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄18-80岁；（2）首次行导管射频消融术，术前行经食道超声或左房肺静脉CTA除外左房内血栓；（3）存在可明确诊断为心房颤动的心电图，即表现为：P波消失，代之以f波，频率350-600次/分，QRS波节律绝对不规则，表现为RR间期不匀齐，QRS波形态多正常；（4）存在心慌、胸闷等症状或应用抗心律失常药物（antiarrhythmic Drugs，AAD）治疗无效；（5）术中行双肺静脉隔离后进行电生理检查，诱发心动过速提示上腔静脉起源或者上腔静脉电位活跃；（6）签署知情同意书。

Inclusion criteria

(1) They were 18-80 years old（ 2) Radiofrequency catheter ablation was performed for the first time, except for left atrial thrombus by transesophageal ultrasound or left atrial pulmonary vein CTA（ 3) There are ECG which can be definitely diagnosed as atrial fibrillation, that is: P wave disappeared, replaced by F wave, frequency 350-600 times / min, QRS wave rhythm is absolutely irregular, RR interval is uneven, QRS wave shape is normal（ 4) There were palpitation, chest tightness and other symptoms, or the application of antiarrhythmic drugs (AAD) was ineffective（ 5) Electrophysiological examination was performed after bilateral pulmonary vein isolation, and tachycardia was induced to indicate the origin of superior vena cava or the potential of superior vena cava was active（ 6) Sign informed consent.

排除标准：

（1）左房内有血栓形成的患者；（2）左心室射血分数<35%，甲状腺功能异常；（3）既往AF射频消融术史、CABG史；（4）严重心脏瓣膜病或人工瓣术后；（5）既往上腔静脉狭窄、窦房结功能异常、起搏器置入术后；（6）左房大于60mm；（7）合并恶性肿瘤等消耗性疾病，预期生存寿命小于1年者；（8）因各种原因不能配合随访者。

Exclusion criteria：

(1) Patients with thrombosis in left atrium（ 2) Left ventricular ejection fraction was less than 35%, thyroid function was abnormal（ 3) Previous history of AF radiofrequency ablation and CABG（ 4) Severe valvular heart disease or after prosthetic valve surgery（ 5) Previous superior vena cava stenosis, sinus node dysfunction, pacemaker implantation（ 6) The left atrium was more than 60 mm（ 7) Patients with consumptive diseases such as malignant tumor and life expectancy less than 1 year（ 8) Those who cannot cooperate with the follow-up for various reasons.
