Objectives of Study：

To investigate the effect of self-management intervention based on Internet and evidence synthesis on elderly patients with chronic obstructive pulmonary disease at stable stage

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合指南诊断标准为COPD且处于稳定期；②年龄≥60周岁；③可配合完成院后随访指导、测评；④患者具备使用智能手机能力；⑤未接受COPD自我管理干预系统指导；⑥自愿参与本研究，签署知情同意书。

Inclusion criteria

① COPD meets the guideline diagnostic criteria and is in stable phase; ② ≥60 years old; (3) Can cooperate with the completion of post-hospital follow-up guidance, evaluation; (4) Patients have the ability to use smart phones; (5) not receiving guidance from the COPD self-management intervention system; (6) Voluntarily participate in the study and sign the informed consent.

排除标准：

①骨科手术；②神经、精神问题致使功能障碍或不能配合者；③不稳定的心脏疾病；④严重肺动脉高压；⑤合并干扰研究的其他肺部疾病症。

Exclusion criteria：

① orthopedic surgery; ② neurological or mental problems resulting in dysfunction or inability to cooperate; ③ unstable heart disease;④ Severe pulmonary hypertension; ⑤Other pulmonary diseases with interference in the study.
