研究目的：

面向原发性肝癌中医个体化诊疗数据，通过真实世界中医案例研究模式，应用倾向评分和工具变量法消除混杂因素对疗效评价的影响，建立循证证据，为临床决策提供依据；通过原发性肝癌中医临床循证证据的构建，为中医大数据的临床研究提供一种新的研究模式。  

Objectives of Study：

In view of the individualized diagnosis and treatment data of TCM for primary liver cancer, through the real world case study model of TCM, the effect of the tendency score and the tool variable method is used to eliminate the influence of the confounding factors on the evaluation of the curative effect, to establish evidence-based evidence and to provide the basis for the clinical decision, and to establish the clinical evidence-based evidence of the primary liver cancer for the large data of traditional Chinese medicine. Bed research provides a new model of research.

药物成份或治疗方案详述：

（1）确立原发性肝癌中医个体化诊疗的分层因素和临床数据的混杂因素 采用文献分析法、专家咨询法确立中医个体化诊疗的分层因素：用药方法分为中成药制剂、辨证应用汤剂及二者联合应用等；用药时间分为全过程用药、间断用药等。采用文献分析法、数据模拟法和专家咨询法等确立数据的混杂因素：如患者年龄、性别、疾病分期、体力活动状态评分、肿瘤状态、肝功能状态等。 （2）应用随访系统获取原发性肝癌患者随访数据 采用病历随访系统对匹配后的患者进行随访，应用数据挖掘方法获取患者年生存率、肿瘤复发率、PS 评分等指标数据，建立临床数据库，运用Cox 比例风险模型对分层进行回归分析。 （3）原发性肝癌中医临床案例数据的混杂因素处理 基于临床电子病历，分别采用倾向性评分法和工具变量法对原发性肝癌中医临床案例数据的混杂因素进行处理，通过数据模拟比对二者效果，确立最优的混杂因素处理方法；运用Cox 比例风险模型对中医个体化诊疗的分层因素进行回归分析。 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

首次就诊，根据BCLC 分期诊断为0 期、A期、B 期、C 期、D期的患者；首次就诊后接受过中医药治疗的患者；同期确诊未接受中医药治疗的患者；就诊时间为原发性肝癌诊疗规范（2011 年版）颁布以后的患者；知情同意，同意接受随访的患者。

Inclusion criteria

1. Patient with primary liver cancer, BCLC stages 0, A, B, C, D, according to the Liver Cancer diagnose standard 2011 version;
2. Patients treated by or not by Chinese medicine after the first diagnosis;
3. The diagnosis was not accepted during the same period.Patients with medical treatment;
4. Informed consent, to be able to complete followed up.

排除标准：

原始病历信息不完整的患者；不接受随访的患者；有精神因素和其它精神行为不全的患者；其它任何因素影响研究数据收集的患者。

Exclusion criteria：

Patients with incomplete information of the original medical records; Patients who were not followed up; Patients with mental and other mental disorders; Any other factor affecting the data collection of patients.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

原发性肝癌真实世界中医临床数据获取分为两部分。一部分是临床电子病历数据，一部分是非住院期间的随访数据。 （1）临床电子病历数据获取方法：基于结构化电子病历的数据： 应用临床科研共享系统的科研数据直接提取技术，从电子病历中直接提取临床数据；没有结构化病历数据，采用电子病历数据检索系统，以“原发性肝癌”为检索词，检索全部住院病历，采用人工录入的方式，获取临床数据。 （2）应用随访系统获取原发性肝癌患者的随访数据

(1)Clinical electronic medical record data acquisition methods: based on the structured electronic medical record data: application of clinical scientific research and sharing system of the scientific research data direct extraction technology, clinical data extracted directly from the electronic medical record; No structured medical record data, using the electronic medical record data retrieval system, and the search term "primary liver cancer, retrieve all hospital medical records, with the method of manually, obtain clinical data. (2) the follow-up data of patients with primary hepatocellular carcinoma were obtained by using the follow-up system.
