研究目的：

本研究拟探讨肺复张手法对妇科腹腔镜手术术后并发症的影响。  

Objectives of Study：

The purpose of this study was to investigate the effect of retraction manipulation on postoperative complications of gynecological laparoscopic surgery.

药物成份或治疗方案详述：

由同一组技术熟练的手术医生实施手术，纳入患者均采用三个相同的腹腔镜切口，术中CO2气腹压均维持在12mmHg。两组均采用容量控制通气模式，VT6～8ml/kg，适当调节呼吸频率维持PETCO2于35～45cmH2O之间。PRM组患者于诱导后每30分钟及术毕放气腹前实施压力控制肺复张手法（每30分钟及放气腹前将PEEP增加到20cmH2O，然后将压力控制水平增加到40～45cmH2O,维持30s）。CV组患者常规机械通气，全程不实施肺复张手法，术毕使用传统方式放气腹。术毕两组均行肺部超声检查。 

Description for medicine or protocol of treatment in detail：

The same group of skilled surgeons perform the operation. The included patients all adopt the same three laparoscopic incisions, and the intraoperative CO2 pneumoperitoneum pressure is maintained at 12mmHg. Both groups adopt volume-controlled ventilation mode, VT6 ~ 8ml/kg, and maintain PETCO2 between 35 ~ 45cmH2O by properly adjusting respiratory frequency. In the PRM group, pressure control is applied every 30 minutes after induction and before peroperatively pneumoperitoneum venting (PEEP is increased to 20cmH2O every 30 minutes and before pneumoperitoneum venting, and the pressure control level is increased to 40-45cmh2o and maintained for 30s). The patients in the CV group receive routine mechanical ventilation, and the retraction technique is not applied during the whole procedure. After the operation, both groups undergo pulmonary ultrasound examination. 

纳入标准：

年龄18～60岁，ASAⅠ～Ⅲ级。

Inclusion criteria

1. Aged 18 to 60 years;
2. ASA I to III.

排除标准：

1.过往有三次以上腹部开放性手术史；
2.腹膜炎、腹结核病史；
3.气胸、肺大疱史；
4.心功能不全、循环系统不稳定、脑损伤；
5.术后随访时间小于2天者。

Exclusion criteria：

1. Patients with more than three previous open abdominal operations;
2. With histories of peritonitis and abdominal tuberculosis;
3. With pneumothorax and bullae of lung;
4. With cardiac insufficiency, unstable circulatory system, and brain injury;
5. Patients with postoperative follow-up less than 2 days.
