研究目的：

本课题旨在通过对活髓切断治疗的临床研究，评估活髓切断治疗在根尖发育成熟恒牙的牙髓损伤中的治疗效果。  

Objectives of Study：

This clinical research tends to evaluate the curative effect of pulpotomy in permanent mature teeth with pulp injury .

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①患者年龄≥18周岁并且无影响口腔治疗的系统性疾病史；
②因龋或磨耗引起的牙髓损伤的恒磨牙，包含可复性牙髓炎及部分不可复性牙髓炎，并且牙根发育完全；
③患牙冷诊敏感，刺激去除后疼痛可在几分钟内缓解，无夜间痛史，口内检查垂直或侧向叩诊均无叩痛；
④剩余牙体量足够进行冠部修复；
⑤牙周组织健康；
⑥X线片示根尖周无明显异常，根尖周指数（PAI）≤1；
⑦临床操作中去冠髓后根髓出血颜色鲜红且冲洗5min内能够止血。

Inclusion criteria

①The patients are over 18 years old and have no history of systemic disease that affect oral treatment.
②Mature permanent molar teeth with pulp injury, involved both reversible pulpitis or partial irreversible pulpitis ,due to caries or abrasion.
③Patientsdescriptions of sensitivity to cold, the pain can be relieved within a few minutes after removing the irritation; No history of night pain; No discomfort in percussion.
④Remaining tooth tissue has enough space for restoration.
⑤Having healthy periodontal tissue.
⑥X-ray film showed no obvious abnormality around the apex of the tooth PAI≤1.
⑦After excavation, red homogeneous and blood filled tissue was observed on the surface of the pulp woundhaemostasis could be achieved within 5 minutes.

排除标准：

①乳牙或根尖未发育完全的年轻恒牙；
②因楔缺、牙隐裂或牙外伤引起的牙髓炎；因龋出现牙髓炎症状的患牙去净龋坏后未见穿髓；
③明显热刺激痛、持续性刺激痛、持续性钝痛、剧烈自发痛、叩诊不适，或者疼痛程度可随体位改变；
④剩余牙体组织量无修复价值；
⑤牙体松动、探及深牙周袋或牙龈红肿见瘘管；
⑥X线示PAI≥2；
⑦去除冠髓后根髓颜色灰白无出血或血液呈暗红色，冲洗5 min后仍无法止血。

Exclusion criteria：

①Deciduous teeth or Immature Permanent Teeth.
②It is caused by wedge-shaped defect, cryptoschisis or tooth fracture. Or after meticulous caries excavation, none exposures appears.
③Sensitive to heat,persistent irritating pain or dull throbbing pain, sharp spontaneous pain and tenderness to percussion or pain exacerbated by lying down.
④Remaining tooth tissue has no restorative value.
⑤Periodontal tissue unhealthy.
⑥PAI≥2.
⑦After excavation,tissue was observed unhealthy on the surface of the pulp woundhaemostasis could not be achieved within 5 minutes.
