研究目的：

探究右美托咪定对老年腹部手术患者术后胃肠道功能（POGF）的影响  

Objectives of Study：

Investigate the effct of dexmedetomidine on the postoperative gastrointestinal function in the elderly abdominal surgery.

药物成份或治疗方案详述：

药物成分：右美托咪定是一种新型高选择性α2肾上腺素能受体激动剂，具有镇静、抗焦虑、催眠、镇痛和交感神经阻滞作用，在围术期具有非常广泛的应用前景。相关系统性研究证实，围术期使用右美托咪定可以稳定血压和心率，从而很好的控制外科术中应激；同时，右美托咪定还能降低术后不良反应的发生如恶心呕吐、躁动、谵妄、寒战等。此外，在动物实验中证实右美托咪定能抑制大鼠胃肠道的排空与排便，也有相关临床研究表明右美托咪定能够改善胃肠道术后患者的胃肠功能。 治疗方案：在麻醉诱导插管前15min，实验组给予负荷量试验药物右美托咪定0.125ml/kg负荷量，泵注15min，之后按照0.05ml/kg/h 速率持续静脉泵注维持，手术结束前30min停止输注。对照组静脉输入等容量的生理盐水作为对照。 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、签署知情同意书；
2、 拟择期全身麻醉腹部手术患者；
3、 年龄在 65-90 岁；
4、 ASA分级 I-III级；
5、 预计手术时间在1-6h内手术。

Inclusion criteria

1. patients who signed a consent form;
2. Patients undergoing selective abdominal surgery under general anesthesia;
3. aged 65-90 years old;
4. ASA classification I-III;
5. operative time 1-6 hours.

排除标准：

1、胃肠道蠕动障碍； 2、有过腹部手术史者； 3、严重的肝肾功能不全者； 4、II-III 度房室传导阻滞者； 5、心率小于 50 次/分者； 6、精神或心理疾病患者； 7、预测可能发生或曾发生困难气道者或术前预计延迟拔管患者； 8、确定/怀疑有滥用麻醉性镇静镇痛药者； 9、存在对试验药物及其他麻醉药物的禁忌。

Exclusion criteria：

1. patients who have gastrointestinal motility disorder;
2. A history of abdominal surgery;
3. Severe hepatic and renal dysfunction;
4. II-III auriculo-ventricular block;
5. heart rate below 50 beats per minute;
6. have mental illness;
7. history of difficult airway or delayed exhubation;
8. Abusing narcotic sedative analgesics;
9. be allergy to dexmedetomidine or other anesthetic drugs.
