研究目的：

胆道闭锁（BA）严重威胁新生儿健康，是导致儿童肝脏移植的首要病因。我们已发表的研究发现以B细胞为核心的自身免疫性肝损伤是BA的重要发病机制。B细胞激活因子（BAFF）激活B细胞的增殖和活化，在BA发病中起重要作用。贝利木单抗目前已被批准应用于治疗自身免疫性疾病红斑狼疮（SLE）。本研究拟探索贝利木单抗治疗 BA 的有效药物剂量及安全性。  

Objectives of Study：

Biliary atresia (BA) is an immune-mediated liver disease and major cause of childhood liver failure and transplantation.We have reported that B cells played a pivotal role in mediating autoimmune liver damage. B-cell activating factors (BAFF), which maintained B cell survival and activation, played a role in BA pathogenesis. Belimumab has became the first FDA-approved agent for treatment of autoimmune disease systemic lupus erythematous (SLE). This study is aimed to test the safety and dosage of Belimumab to treat patients with BA.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.本研究的受试者在广州市妇女儿童医疗中心肝胆外科由经验丰富的医生诊断高度怀疑BA的患者中招募，患者有计划接受肝胆手术；
2. 0-3 个月龄梗阻性黄疸患儿；
3.经术中胆道造影明确诊断 BA；
4.造血功能正常；
5.监护人知情同意。

Inclusion criteria

1. The subjects of this study were recruited from the Hepatobiliary Surgery Department of Guangzhou Women and Children's Medical Center who were diagnosed with highly suspected BA by experienced doctors, and the patients were planned to undergo hepatobiliary surgery;
2. 0-3 months old children with obstructive jaundice;
3. A clear diagnosis of BA by intraoperative cholangiography;
4. Normal hematopoietic function;
5. The guardian's informed consent.

排除标准：

1.合并全身炎症反应综合征或多系统畸形的患儿；
2.原发疾病诊断不明确的患儿。

Exclusion criteria：

1. Children with systemic inflammatory response syndrome or multiple system malformations;
2. Children with unclear diagnosis of the primary disease.
