研究目的：

探讨雷火灸治疗肾虚血瘀型前列腺增生术后下尿路症状的临床疗效，为前列腺增生的治疗提供临床研究依据。  

Objectives of Study：

To investigate the clinical effect of thunder and fire moxibustion on the lower urinary tract symptoms after the treatment of Benign Prostatic Hyperplasia with kidney deficiency and blood stasis, and to provide clinical research basis for the treatment of Benign prostatic hyperplasia.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）符合本病诊断标准
①50岁≤年龄≤85岁的男性患者；
②临床症状：尿频、尿急、排尿费力、尿滴沥、夜尿次数增多、排尿困难严重者，可伴有尿失禁或尿潴留；
③査体直肠指诊或前列腺彩超提示前列腺体积增大。
④辅助检査提示：尿流率提示最大尿流率＜15ml/s；经腹壁超声测定膀胱残余尿量＞50ml。具备以上①、②、③项或者兼有第④项者，可诊断为前列腺增生伴下尿路症状。
（2）中医辨证诊断参照：符合肾虚血瘀型精癃诊断:
①主症：尿后滴沥、尿频、排尿困难、遗尿或失禁、夜尿增加；
②次症:神疲乏力，小腹部、会阴部、耻骨区或腰骶及肛周疼痛，腰膝酸软，畏寒怕冷；
③舌脉:舌暗淡或有瘀点、有瘀斑，苔薄白，脉沉涩。
具备主症２项及次症2项（舌象和脉象必须具备之一），可诊为肾虚血瘀型精癃。
（3）诊断前列腺增生症且行手术治疗后出现下尿路症状者；
（4）自愿参加并签署知情同意书的患者。

Inclusion criteria

Meet the diagnostic criteria of this disease:
① Male patients ≤ 50 years old ≤85 years old;
②Clinical symptoms: frequent urination, urgent urination, laborious urination, urine dripping, increased nocturnal urination times, severe urination difficulties, may be accompanied by urinary incontinence or urinary retention;
③digital rectal examination or prostate color ultrasound indicate enlarged prostate volume;
④ Auxiliary examination suggested that the maximum urinary flow rate was<15ml/s; Residual urinary volume＞50ml was determined by abdominal wall ultrasound. Patients with the above items ①, ②, ③ or both the fourth item can be diagnosed as prostatic hyperplasia with lower urinary tract symptoms.
TCM Diagnosis Reference: in line with the kidney deficiency and blood stasis type of Jinghuang diagnosis:

(1) Main symptoms: dripping after urine, frequency of urination, dysuria, enuresis or incontinence, nocturia increased;

Second disease: fatigue, small abdomen, perineum, pubic area or lumbosacral and perianal pain, waist and knee acid and soft, fear of cold;
(3) tongue pulse: faint tongue or petechiae, petechiae, thin white moss, pulse heavy astringent.
2 main symptoms and 2 secondary symptoms (one of tongue and pulse symptoms is required), and it can be diagnosed as a miracle-related disease concerning kidney deficiency and blood stasis.
(3) Patients diagnosed with prostatic hyperplasia and with lower urinary tract symptoms after surgical treatment;
(4) Patients who voluntarily participated and signed the informed consent.

排除标准：

（1）因泌尿系肿瘤、尿路结石、肾功能衰竭所导致的无尿或者少尿；
（2）因尿道狭窄或尿路感染或输尿管畸形等其他原发疾病导致的排尿障碍；
（3）近1个月内服用过可能影响膀胱出口功能的药物；
（4）合并急性或严重的心脑血管疾病、肺肝肾功能不全、精神类疾病、不能配合治疗者；
（5）皮肤感染、皮肤病损、创伤及肿瘤患者、不能行雷火灸者；
（6）正在参与其他试验者。
（7）对头孢菌素类过敏者。

Exclusion criteria：

(1) Anuria or oliguria caused by urinary tract tumor, urinary calculus, and renal failure;
(2) urination disorders caused by urethral stricture or other primary diseases such as urinary tract infection or ureteral malformation;
(3) have taken drugs that may affect the bladder outlet function in the past 1 month;
(4) patients with acute or severe cardiovascular and cerebrovascular diseases, lung and liver and kidney insufficiency, mental diseases, and cannot cooperate with treatment;
(5) patients with skin infection, skin lesions, wounds and tumors, and those who cannot perform thunder and fire moxibustion;
(6) Other participants are participating in the experiment.
(7) People allergic to cephalosporins.
