研究目的：

本项目预期将提交具有中医特色的针对衰弱老年人的综合评估工具、针对衰弱老年人综合评估和多学科管理模式的操作手册、以及该模式改善衰弱老年人预后的评价报告，项目实施后将有利于推出改善衰弱老年人预后的新模式和新举措，减轻衰弱老年人对社会、家庭、医疗的负担，具有良好的科学价值和社会、经济学效益。  

Objectives of Study：

The project is expected to submit a comprehensive evaluation tool for the weak elderly with traditional Chinese medicine characteristics, an operation manual for the comprehensive evaluation and multidisciplinary management model for the weak elderly, and an evaluation report on the improvement of the prognosis of the weak elderly by this model. After the implementation of the project, it will help to promote new models and measures to improve the prognosis of the weak elderly and reduce the impact of the weak elderly on the society The burden of family and medical treatment has good scientific value and social and economic benefits.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

选取中国中医科学院西苑医院老年病一科、老年病二科病人，采用FRIED（改良版）量表进行评估，筛选符合衰弱诊断标准的患者340例入组。
(1)年龄≥65岁；
(2)符合以下情况中的任意3项者：
FRIED
1.1年内体重减轻＞3kg或5%
2.自觉疲惫，上周内超过3天，有一问为“是” （1）我做任何事都觉得费劲
（2）缺乏干劲
3.肌力下降：握力（kg）取决于性别和IBM 男≤29—32；女≤17—21
4.躯体功能下降：步速减慢（1.5ft或4.5m） 男≤173cm，≥7s；＞173cm， ≥6s
女≤159cm，≥7s；＞159cm， ≥6s
5.躯体活动量降低 男＜383kcal/wk；女＜270kcal/wk

Inclusion criteria

The inpatients of the first Department of geriatrics and the second Department of geriatrics in Xiyuan Hospital of Chinese Academy of traditional Chinese medicine were evaluated by friend (Taiwan improved version) scale and frailty index (FI), and 340 patients who met the diagnostic criteria of frailty were selected.

(1) Age ≥ 65 years;

(2) Any 3 of the following conditions are met:

FRIED

1. Weight loss within 1 year ＞ 3kg or 5%

2. Self conscious fatigue, more than 3 days last week, one question is "yes" (1) I find it hard to do anything

(2) Lack of motivation

3. Decreased muscle strength: grip strength (kg) depends on gender and IBM Male ≤ 29-32; Female ≤ 17-21

4. Decreased physical function: slow down the pace (1.5ft or 4.5m) Male ≤ 173cm, ≥ 7S; ＞ 173cm ≥6s

Female ≤ 159cm, ≥ 7S; ＞ 159cm ≥6s

5. Decreased physical activity Male ＜ 383kcal / wk; Female ＜ 270kcal / wk

排除标准：

（1）患者拒绝接受衰弱筛查；
（2）患者无法进行交流或现有临床资料无法获得有效信息来判断患者衰弱与否；
（3）严重认知功能障碍；
（4）严重肝肾功能不全以及其它损害患者认知及活动能力的疾病终末期；
（5）主要研究者认为不适合参与本项研究的任何其它情况或事件。

Exclusion criteria：

(1) The patient refused to accept weakness screening;

(2) The patient is unable to communicate or the existing clinical data are unable to obtain effective information to judge whether the patient is weak or not;

(3) Severe cognitive impairment;

(4) The end stage of severe liver and kidney dysfunction and other diseases that impair the cognitive and activity of patients;

(5) Any other circumstances or events that the principal investigator considers inappropriate to participate in this study.
