研究目的：

本研究主要针对急诊非肿瘤开颅手术术后创面出血。通过术后强化血压管理，术后超早期行颅脑CT检查，旨在最大限度减少手术创面再出血，减少术后发生脑疝几率。特别是减少非计划二次手术率，增加手术成功率，提高手术效果，提升患者术后生存质量。  

Objectives of Study：

This study is mainly aimed at the wound bleeding after emergency non tumor craniotomy. Through strengthening the management of blood pressure after operation and early brain CT examination after operation, the aim is to minimize the rebleeding of surgical wound and reduce the probability of postoperative cerebral hernia. In particular, it can reduce the rate of unplanned secondary surgery, increase the success rate of surgery, improve the effect of surgery, and improve the quality of life of patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄＞16岁、＜80岁；
2.符合以下标准的具有外科开颅手术指征，伴有血压升高患者（收缩压≥150 mmHg）：
（1）.颅脑创伤需行开颅血肿清除术；
(1) （2）.基底节区、丘脑出血：①基底节血肿量超过30ml，颞叶血肿量超过20ml；②颞叶钩回疝；③影像学有明显颅内高压表现(中线结构移位超过5 mm；同侧侧脑室受压闭塞超过1／2；同侧脑池、脑沟模糊或消失)；
3.无其他严重疾病，严重影响患者预后情况。（高血压病除外）
4.本次发病首次行开颅手术，本次手术入路部位无手术史。

Inclusion criteria

1. Age > 16 and < 80; 2. Patients with high blood pressure (systolic blood pressure ≥ 150 mmHg) and indications of craniotomy according to the following criteria: (1) craniotomy hematoma evacuation is necessary for craniocerebral trauma（ 1) （ 2) Hemorrhage in basal ganglia and thalamus: ① the volume of hematoma in basal ganglia and temporal lobe exceeded 30ml and 20ml respectively; ② Uncinate hernia of temporal lobe; ③ Imaging findings showed obvious intracranial hypertension (midline structure shift more than 5 mm; More than 1 / 2 of the ipsilateral ventricle was compressed and occluded; The ipsilateral cistern and sulcus were blurred or disappeared; 3. No other serious diseases, which seriously affect the prognosis of patients（ Except hypertension) 4. Craniotomy was performed for the first time in this case, and there was no operation history in this operation.

排除标准：

1.术前双侧瞳孔散大，生命体征不稳；
2.有血液系统疾病、长期口服抗凝药物凝血功能障碍；
3.术后病情持续恶化，生命体征不稳，无法行颅脑CT检查；
4.术后确诊断为继发性恶性高血压患者；
5.术前证实为肿瘤卒中、脑动脉瘤破裂、脑血管畸形出血；
6.单纯性创伤性硬膜外血肿。

Exclusion criteria：

1. Dilated pupil and unstable vital signs before operation; 2. Blood system diseases, long-term oral anticoagulant drug coagulation dysfunction; 3. The patient's condition continued to worsen after the operation, and the vital signs were unstable, so the brain CT examination could not be performed; 4. The patients were diagnosed as secondary malignant hypertension after operation; 5. It was confirmed as tumor apoplexy, rupture of cerebral aneurysm and hemorrhage of cerebral vascular malformation before operation; 6. Simple traumatic epidural hematoma.
