本课题拟研究脑多靶点关联刺激技术及多靶点电磁刺激聚焦技术,克服既往单一靶点刺激的缺点,研制磁共振成像导航的经颅磁刺激设备;同时基于多模态量化脑功能网络特征,实现同步及交替刺激脑皮层网络关键节点;将不同作用机制且有协同效应的重复经颅磁刺激和经颅直流电刺激技术相结合,提高刺激效能,集成皮层自调控系统(Cortical auto-modulation system,CAMOS),改进神经调控治疗效果,最终实现从根本上改变意识障碍患者预后的治疗目标。 |

Objectives of Study: |
This topic to study brain targets associated with stimulation technology and multiple targets more electromagnetic stimulation is focused on the technology, the disadvantages of ever single target stimulus transcranial magnetic stimulation in magnetic resonance imaging (fmri) navigation equipment developed; At the same time, the key nodes of the cortical network are stimulated by synchronous and alternating stimulation based on multi-modal quantitative brain function network characteristics. Different mechanism of action and has synergistic effect of repetitive transcranial magnetic stimulation combined with transcranial direct current stimulation technology, increase the efficiency of the stimulation and integrated cortex self regulation system, improve the neural regulation of therapeutic effect, finally realizes the fundamentally change the prognosis of patients with disturbance of consciousness of treatment goals. |

纳入标准: |
昏迷:(1)性别不限,年龄18-65岁;(2)严重脑损伤导致昏迷(包括创伤、脑血管病、缺血缺氧性脑病或其他严重脑损伤)患者;(3)昏迷持续一周以上;(4)影像学检查(头MRI)无明显脑结构的移位、缺失、大片坏死改变及明显的脑干(不包括锥体束)或丘脑病灶,各脑叶病灶范围不能超过一侧该脑叶范围的30%;(5)病情稳定,生病体征平稳;(6)患者家属自愿参加本项研究,并签署知情同意书。 |

排除标准: |
(1)评估期间使用镇静安眠药、麻醉药、精神药物、肌肉松弛剂或者Na+、Ca++通道阻滞剂(如卡马西平等);(2)呼吸机依赖;(3)病程1年以上;(4)存在电磁相关检查、治疗的禁忌,包括体内含有金属材料(如起搏器、假牙、钢钉等)、开放性颅脑损伤或颅骨缺损而无法行电磁刺激者;(5)既往有癫痫病史,临床提示癫痫发作或脑电图有痫样放电的患者;(6)合并其他系统严重疾病(如心、肝、肾等功能衰竭)者;(7)有中枢神经系统肿瘤或变性病等进行性加重的神经系统疾病。 |
