研究目的：

一、总体目标 评价有氧联合抗阻力运动训练对轻度老年糖尿病周围血管病变（PAD）干预的效果。 二、具体目标 1.描述轻度老年糖尿病PAD患者下肢动脉血流情况、行走能力及生活质量等情况，筛查轻度老年PAD患者； 2.评价有氧联合抗阻力运动训练对改善轻度老年糖尿病PAD患者下肢动脉血流情况、行走能力及生活质量等的影响。  

Objectives of Study：

General ObjectiveTo evaluate the effect of combined exercise for the elderly patients with mild diabetic peripheral arterial disease. Specific objectives: 1. Describe the blood flow condition, walking ability and quality of life of the elderly patients with mild PAD, and screen patients with mild PAD; 2. Evaluate the effects of combined exercise on the blood flow condition, walking ability and quality of life of the elderly patients with mild PAD.

药物成份或治疗方案详述：

有氧运动结合抗阻力运动训练：1、训练方式和频率：有氧运动训练借助手摇、脚踏一体化功率车作为训练辅具，方法为20min/次，以30rpm的速度转2min后休息1min，不断重复。抗阻力运动训练借助弹力带作为训练辅具，方式包括包括弹力带直立拉伸、反向蜷腹、站姿肩外旋、直臂侧屈伸、颈后肘伸展、肘屈曲、站姿抬腿、侧立提拉、站姿后踢腿、站立下蹲、上拉弹力带弓箭步、横拉转体、双臂上举深蹲，30min/次。2、训练时间：联合运动训练3次/周，每次60min左右，按“运动前热身（左右扭胯、扭膝、绕踝、原地踏步）5min-有氧运动20min-休息10min-抗阻力运动20min-运动后放松5min”的顺序进行，共12周。3、训练强度：遵循循序渐进原则，注意观察患者情况，使患者心率始终保持在每分钟130次以内，Borg主观劳累程度评分在13-14分（有点用力，稍感费力）左右。4、安全保障：干预方案经过专家论证安全性、有效性后才开展干预；干预场所有一间可专门用于患者的运动训练的活动室，采光凉好、环境适宜、基础设施齐全，可为患者提供良好的训练环境；监督与指导：有专门的糖尿病专科护士，可在每周自主训练时协助监督、指导；开始接受训练时，不强求达到所测标准，可以逐步适应；运动训练期间如出现感冒或其他疾病，应停止训练，对症处理，完全恢复后，重新开始训练时仍要从小运动量开始；每次询问患者睡眠、食欲、精神状态是否正常；运动全程检测您的血氧饱和度、心率、呼吸、血糖情况，并备有糖果、水等，训练全程有全科医生在场应对突发事件，一旦有任何问题均会及时处理。 

Description for medicine or protocol of treatment in detail：

The aerobic exercise combined with resistance exercise training is used as the intervention program.1. Training mode and frequency: Arm-leg-ergometry cycle is used to do the aerobic exercise training; training 20min each time, and 2min at 30rpm with 1min rest and repeat. Elastic belt is used to do the resistance exercise training. Training methods include vertical stretching, reverse curled abdomen, standing shoulder extorsion,straight arm flexion, elbow flexion and extension behind the neck, standing squat, side vertical lift, standing after the kick stand, squat, pull on the elastic belt lunge, horizontal swivel, lift the arms squat;30min each time.2. Training time: 12weeks training,3 times a week, about 60min each time with warm-up 5min-aerobic exercise 20min-rest 10min-resistance exercise 20min-cold down 5min.3.Training intensity:Following in order and advance step by step.Make sure that the patient's heart rate is always kept at 130 times per minute, and the score of the Borg subjective fatigue is 13-14 (feeling a little hard).4.Safety guarantee:Before intervention, the programme must be proved to be safe and effective. There is an activity room which can be used specifically for the exercise training. It can provide a good training environment for patients with the warm, convenient environment and complete infrastructure. Supervision and guidance: Diabetes care specialists can assist in supervision and guidance during the weekly training.When the patients start to receive training, they neednt to meet the criteria,you can adapt to it gradually.If the patient has a cold or the other disease,he/she should stop training for the treatment.When fully recovered,he/she restart training with small intensive exercise.Their sleep,appetite and mental state should be focused on.The blood oxygen saturation, heart rate, respiration and blood glucose should be tested.The candy and water should be prepared.The general practitioner should be present to deal with emergencies.  

纳入标准：

年龄60-75岁；2型糖尿病，符合1999年（WHO）2型糖尿病诊断标准；经过DSA或下肢血管彩色B超检测，证实存在轻度下肢血管病变；自愿参加本研究，签署知情同意书。

Inclusion criteria

60-75 years old; Type 2 diabetes, consistent with the diagnostic criteria in 1999 (WHO); confirmed mild lower limb vascular disease after DSA or lower limb blood vessel color ultrasound detection;voluntary participation in this study, signed informed consent.

排除标准：

自身胰岛素严重分泌不足的1型糖尿病；血糖极不稳定的糖尿病；血压过高，收缩压大于180mmHg；血糖过高，大于14mmol/L；糖尿病严重下肢血管病变（足部溃疡与坏疽）；截肢；有严重心肺功能不全、急性感染、肾脏并发症及严重视网膜病变；经常有脑供血不足；多发性大动脉炎、严重下肢静脉曲张等其他周围血管疾病；存在严重骨骼肌肉疾病；有高危跌倒倾向；无法独立完成本运动方案；认知功能障碍。

Exclusion criteria：

Type 1 diabetes with severe insulin secretion;Diabetes with unstable blood glucose; High blood pressure, systolic pressure is higher than 180mmHg; High blood glucose is higher than 14mmol/L; Diabetic severe lower limb angiopathy (foot ulcer or gangrene); Amputation;Severe cardiopulmonary insufficiency, acute infection, renal complications and severe retinopathy; insufficient blood supply to the brain; Multiple arteritis, severe lower limb varicose veins and other peripheral vascular diseases; serious skeletal muscle diseases; Prone to fall; Unable to complete the program independently; Cognitive dysfunction.
