研究目的: |
肾积水是儿童泌尿系统最常见的先天畸形之一。目前,临床上对于婴儿肾积水的干预策略缺乏共识。本研究拟采用多中心、前瞻性、随机对照的方式,评估目前临床中常用的干预时机与方式对患儿肾功能的保护效力,为婴儿肾积水的干预时机和方式判断提供高级别临床证据,从而使更多的患儿能够得到最佳的诊疗方案。 |

Objectives of Study: |
Hydronephrosis is the most common congenital abnormities. There is still no consensus on the intervene strategy. This prospective, multi-center, randomized, controlled trial aim to evaluate the effect of clinical major intervene strategy for protecting renal function, so that could provide high level clinical evidence for a better intervene scheme. |

药物成份或治疗方案详述: |
1) 建立多中心队列前瞻性收集符合标准的新生儿肾积水患儿的基线资料按照随机方法将影像学和肾功能检查结果为重度肾积水A组的患儿分别分入早期手术组或主动监测组。主动监测过程中如果患儿APD进行性增加,或肾功能进行性下降则及时进行手术干预。比较两组患儿术后半年及主动监测1年后的肾功能情况(肾皮质厚度、肾长轴长度、eGFR、血肌酐、APD)。 (2)建立多中心队列,前瞻性收集符合标准的新生儿肾积水患儿的基线资料,按照随机方法将影像学和肾功能检查结果为重度肾积水B组患儿分别分入早期手术或延期手术组。延期手术的处理策略为超声引导下经皮肾造瘘至半岁后再进行手术。比较患儿术后半年及主动监测1年后的肾功能情况(肾皮质厚度、肾长轴长度、eGFR、血肌酐、APD)。 |

Description for medicine or protocol of treatment in detail: |
1.Establish multi-center cohort, prospectively collect infant hydronephrosis baseline information, randomized allocate severe hydronephrosis group A patients into early pyeloplasty or active surveilliance group. If the disease progression happened, pyeloplasty will be performed. Then we evaluate the renal function after 1 year. 2.Establish multi-center cohort, prospectively collect infant hydronephrosis baseline information, randomized allocate severe hydronephrosis group Bpatients into early pyeloplasty or delayed pyeloplasty group. The delayed pyeloplasty group patient will receive nephrostomy firstly and be operated after the children reach half year. Then we evaluate the renal function after 1 year. |

2022年1月-2024年12月就诊于研究相关医疗中心的年龄≤3月龄的肾积水患儿。同时满足如下条件的进入重度肾积水A组别: |

All the patients less than 3 month old in our centers will be included. |

① 存在后尿道瓣膜等下尿路梗阻性疾病 |

对重度肾积水A组和重度肾积水B组各自160例患儿,由研究者采用区组随机化的方法按照1:1的比例分别随机分入两种不同处理单元。区组长度为4,结合随机数字表中取随机数字确定患者组别。区组随机化软件为R(4.0版)。 |
