研究目的：

1.分析不同程度认知功能障碍患者口腔微生态的特点，明确阿尔茨海默病与牙周炎的相关性；2.通过外泌体多组学分析，探索在 AD 进程中起关键作用的分子标志物，为AD的早期诊断和早期治疗提供科学依据  

Objectives of Study：

1. To analyze the characteristics of oral microecology in patients with cognitive impairment, and clarify the correlation between Alzheimer's disease and periodontitis; 2. To explore the molecular markers that play a key role in the process of AD through exosome multiomics analysis, so as to provide scientific basis for early diagnosis and treatment of AD.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①阿尔茨海默病入组标准：在满足National Institute on Aging–Alzheimer’s Association (NIA-AA)： (1)主诉记忆减退，且有知情人证实； (2)根据临床检查，可作出痴呆诊断，通过认知量表测试验证(20项ADL>26分；CDI>1分)；(3)无意识障碍；(4)全部病例均行头颅MRI检查以及相关实验室检查除外可导致记忆力和认知功能进行性损害的其他疾病； (5)淀粉样蛋白/p-Tau在PET示踪中增加或脑脊液Aβ1-42增加伴/或P-Tau增加。
②轻度认知障碍入组标准：(1)以记忆力减退为主诉，且由知情人所证实；(2)客观检查发现有与年龄和受教育水平不相称的轻度认知损害； (3)总体认知功能正常，日常生活基本能力正常，复杂的日常或社会功能可轻微损害； (4)没有痴呆(CDR—O．5分，20项ADL<26分)； (5)筛选1年内无感染、梗塞或其他局灶性脑损伤的证据，也无涉及神经疾病的提示性临床症状；一个非关键性脑区的腔隙性病变且不被认为影响了研究对象的认知功能可包括在本研究中； (6)有足够可配合完成神经心理学测试的视觉和听觉分辨力； (7)病程在3个月以上。
③认知功能正常组入组标准：（1）简易精神状态检查表 (MMSE) >27/30分和正常神经心理评估；（2）临床痴呆评分(CDR)得分= 0；（3）无神经或精神疾病史；（4）无记忆抱怨或认知缺陷。

Inclusion criteria

①Eligibility criteria for Alzheimer's disease: meeting the National Institute on Aging -- Alzheimer's Association (NIA-AA) : (1) Chief complaint of memory loss, confirmed by an informed person;(2) According to the clinical examination, the diagnosis of dementia can be confirmed by the cognitive scale test (20 ADL> score 26;CDI > 1);(3) unconscious disorders;(4) All patients underwent head MRI examination and related laboratory examination except for other diseases that may lead to progressive impairment of memory and cognitive function;(5) Increase in amyloid /p-Tau in PET or increase in cerebrospinal fluid Aβ1-42 with/or p-Tau increase.
(2) Inclusion criteria for mild cognitive impairment :(1) The chief complaint was memory loss, which was confirmed by an informed person;(2) objective examination found mild cognitive impairment not commensurate with age and education level;(3) The general cognitive function is normal, the basic ability of daily living is normal, and the complex daily or social function may be slightly impaired;(4) No dementia (CDR -- O. 5, 20 ADL<26);(5) Screening no evidence of infection, infarction or other focal brain injury, and no suggestive clinical symptoms involving neurological diseases within 1 year;A lacunar lesion in a non-critical brain region not thought to affect the subject's cognitive function could be included in the study;(6) visual and auditory discrimination sufficient for neuropsychological testing;(7) The course of disease is more than 3 months.
③ Inclusion criteria of normal cognitive function group :(1) Simple Mental State Checklist (MMSE) >27/30 score and normal neuropsychological assessment;(2) Clinical dementia score (CDR) = 0;(3) No history of neurological or psychiatric disease;(4) No memory complaints or cognitive defects.

排除标准：

（1）急性感染；（2）CT/MR不耐受，或有下列任何一项病史；（3）严重精神障碍（例如严重抑郁症）；（4）神经障碍；（5）头部受伤；（6）自身免疫性和系统性炎症性疾病（如系统性红斑狼疮、类风湿性关节炎或克罗恩病）；（7）酗酒或滥用药物。近期接受皮质激素或非甾体抗炎药（NSAID）或苯二氮卓类药物治疗

Exclusion criteria：

(1) Acute infection;(2) CT/MR intolerance or a history of any of the following;(3) severe mental disorders (e.g., severe depression);(4) neurological disorders;(5) head injury;(6) autoimmune and systemic inflammatory diseases (such as systemic lupus erythematosus, rheumatoid arthritis, or Crohn's disease);(7) Alcoholism or drug abuse.Recent treatment with corticosteroids or nonsteroidal anti-inflammatory drugs (NSAIDs) or benzodiazepines
