研究目的：

1. 主要目的： 探索在血流动力学不稳定的休克患者MAP/NEQ指数阈值指导早期肠内营养启动的可行性与安全性。 2. 次要目的： （1）获得关于该方法安全性的初步数据，检测随访确定的、潜在的、以患者为中心的主要和其他重要的次要结局指标，以及不良事件报告。 （2）利用该试验数据为后续可能进行的更大规模临床试验的设计提供信息。  

Objectives of Study：

1. Primary Purpose: To explore the feasibility and safety of MAP/NEQ index in guiding early enteral nutrition initiation in patients with shock under hemodynamically unstable. 2. Secondary Purpose: (1) To obtain preliminary data on the safety of the approach, examine potential, patient-centered primary and other important secondary outcomes, and report adverse events. (2) To inform the design of larger clinical trials that may follow.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

预期ICU住院时间>24小时； 诊断为休克的成人患者（>18y）且接受至少一种静脉输注血管活性药物

Inclusion criteria

1. expected to stay in ICU more than 24h; 2. adult patients(>18y) with shock and receiving at least one vasopressor infusion.

排除标准：

对肠内营养配方使用存在禁忌症（包括活动性消化道出血、肠梗阻、肠系膜缺血、胃肠道感染等） 对肠内营养配方的任何成分过敏 具体营养需求：已存在长期家庭肠内营养支持或肠外营养； 孕妇

Exclusion criteria：

1. contraindication for enteral nutrition (including active gastrointestinal bleeding, bowel obstruction, mesenteric ischemia, gastrointestinal infection); 2. allergy to any ingredient of enteral nutrition; 3. pre-exist specific nutrition needs: long-term family enteral nutrition or parenteral nutrition; 4. pregnant.

参与者将被随机选择进入由主治医生密封的不透明信封的两组(1:1比例)中的一组。隐式分配方法是将受试者分别随机分为早期肠内营养组和延迟肠内营养组，并将不透明的密封信封按顺序编号，信封里有一张折叠的纸，上面标有“早期肠内营养”或“延迟肠内营养”。
