研究目的：

1.通过肺超声检查评估以驱动压引导的个体化PEEP滴定策略对Trendelenburg体位腹腔镜手术老年患者肺不张的影响；探讨驱动压引导的个体化PEEP滴定策略对Trendelenburg体位腹腔镜手术老年患者呼吸功能及术后肺部并发症的影响。 2.探讨以驱动压引导的个体化PEEP滴定策略联合间断肺复张是否优于单独使用个体化PEEP。  

Objectives of Study：

1.Lung ultrasound was used to evaluate the impact of a driven pressure-guided individualized PEEP titration strategy on elderly patients with atelectasis under Trendelenburg position laparoscopic surgery; to explore the impact of a driven pressure-guided individualized PEEP titration strategy on the respiratory function of elderly patients with Trendelenburg position laparoscopic surgery The impact of postoperative pulmonary complications. 2.To explore whether an individualized PEEP titration strategy guided by driving pressure combined with intermittent lung recruitment is better than individualized PEEP alone.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄在60至80岁之间；
（2）ASA分级I ～III；体重指数（BMI）＜35 kg/m2；
（3）预计手术期间气腹时间＞2小时，且在Trendelenburg体位下行腹腔镜手术（包括但不限于腹腔镜下结直肠根治术、腹腔镜下膀胱癌根治术）的患者。

Inclusion criteria

Be between 60 and 80 years old;
ASA classification I ~ III; body mass index (BMI) <35 kg/m2;
Patients who are expected to have pneumoperitoneum time> 2 hours during the operation and undergo laparoscopic surgery (including but not limited to laparoscopic radical colorectal resection, laparoscopic radical bladder cancer resection) in Trendelenburg position

排除标准：

（1）病态肥胖(BMI≥35kg/m2)；
（2）既往有胸外科手术史、气胸、肺大疱、严重慢性阻塞性肺疾病病史；
（3）4 周内有上呼吸道或肺部感染史；有胸腔积液；
（4）术前呼吸空气情况下SpO2＜90%或吸氧条件下SpO2＜95%；
（5）患者有严重的心、脑、肝、肾以及神经肌肉功能障碍等疾病；
（6）肺超声图像显影不清晰；
（7）拒绝签署知情同意书。

Exclusion criteria：

(1) Morbid obesity (BMI≥35kg/m2);
(2) Past history of thoracic surgery, pneumothorax, bullae, severe chronic obstructive pulmonary disease;
(3) A history of upper respiratory tract or lung infection within 4 weeks; a pleural effusion;
(4) SpO2＜90% under preoperative air breathing conditions or SpO2＜95% under oxygen inhalation conditions;
(5) The patient has serious diseases such as heart, brain, liver, kidney, and neuromuscular dysfunction;
(6) The lung ultrasound image is not clear;
(7) Refuse to sign the informed consent.
