研究目的：

优化继发性胆总管结石患者治疗方案策略的研究，主要研究在符合一定条件（胆囊管直径较粗，结石较小）的继发性胆总管结石患者中，比较经胆囊管的LCBDE+LC与ERCP+LC的安全性、有效性及经济性。  

Objectives of Study：

The objective of the study is to optimize the treatment strategy for patients with secondary choledocholithiasis, comparing the safety, effeciency, and cost-effectiveness between LTCBDE+LC and ERCP+LC for the secondary choledocholithiasis patients who meet certain conditions (the diameter of the cystic duct is larger and the common bile duct stones are smaller)

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、 符合继发性胆总管结石临床诊断，影像学证据证明胆总管结石并合并胆囊结石。
2、 年龄18-65岁，性别不限，精神正常具备完全民事行为能力。
3、 同意并书面签署研究知情同意书，相关治疗、麻醉及手术同意书，接受根据随机分组所制定的治疗方案。
4、 基线实验室检查符合以下标准：血清ALT、AST ≤ 3×正常值上限（ULN）；TBIL≤85μmol/L；血清肌酐≤ 1.5×ULN；INR < 1.5；凝血酶原时间< ULN+4秒；血小板≥100×10*9/L。
5、 术前影像学测量：胆囊管汇入部直径>5mm（能通过胆道镜），胆总管最大结石的最小直径<5mm（能通过胆囊管取出）。
6、 腹腔镜及ERCP手术指征明确，无手术禁忌症 （ASA评分≤ 3）。

Inclusion criteria

1.Meet the diagnostic criteria of secondary choledocholithiasis, imaging examination confirmed choledocholithiasis and gallstones.
2.Age between 18-65, male of female, mentally normal with full capacity for civil conduct.
3.Agree and sign the research informed consent form, relevant treatment, anesthesia and surgery consent form, and accept the random grouping.
4.Baseline laboratory tests meet following criteria: ALT and AST≤3 times upper limit of normal(ULN), TBIL≤85μmol/L, serum ceratinine≤1.5 times ULN, prothrombin time 5.Preoperative imaging measurements: diameter of the confluence of the cystic duct>5mm, minimal diameter of the largest stone<5mm.
6.Clear indiction for laparoscopy or ERCP, no contraindications(ASA grade≤3).

排除标准：

1、 任何影像学证据证明：胆囊管汇入部直径<5mm，胆总管结石最小直径>5mm，胆囊三角解剖存在可疑变异，肝内胆管结石。
2、 凝血障碍病史；血友病或出血倾向；接受抗凝治疗。
3、 术前任何影像学检查显示肝内胆囊、肝硬化、肝肿块或脓肿。
4、 术前任何影像学检查证据有化脓性或坏死性胆囊炎、胆囊积脓或穿孔的。
5、 术中及术后诊断不符合继发性胆总管结石者。
6、 既往上腹部手术史，既往ERCP、EST操作史。
7、 以任何原因中转开腹手术患者。
8、 病态肥胖(体重指数BMI> 40.0)。
9、 怀孕、产后28天内或哺乳的女性患者。
10、 需要持续免疫抑制剂治疗或全身皮质类固醇。
11、 严重间质性肺炎、肺纤维化或严重肺气肿。
12、 不受控制的糖尿病。
13、 严重的全身器官功能障碍(慢性肝、肾、心脏病)。
14、 HIV阳性。
15、 ASA得分> 3。
16、 仅行ERCP而未行LC的患者。

Exclusion criteria：

1.Any imaging examination proved that:diameter of the confluence of the cystic duct<5mm, minimal diameter of the largest stone>5mm, suspicious anatomy variation of the clot's triangle, intrapheatic bile duct stones.
2.Coagulopathy, hemophilia or bleeding tendecy, receiving any anticoagulant therapy.
3.Any properative imaging examination proved that: Intrahepatic gallbladder, cirrhosis, liver tumor or abscess.
4.Any properative imaging examination proved that: Suppurative or gangrenous cholecystitis, gallbladder empyema or perforation.
5.perioperative diagnosis is not secondary choledocholithiasis.
6.history of upper abdominal surgery, ERCP or EST.
7.conversion to open surgery due to any reasons.
8.Morbid obesity (BMI>40.0)
9.Female patients with pregnancy or breastfeeding.
10.Patients receiving immunosuppressive therapy or systemic corticosteroids.
11.Sever interstitial pneumonia, pulmonary fibrosis or severe emphysema.
12.Uncontrolled diabetes.
13.severe systemic organ dysfunction
14.HIV postive.
15.ASA grade > 3.
16.Patients who only undergo ERCP but not LC.
