研究目的：

通过对临床标本的检测及观察，分析并阐述空气细颗粒沉积对肺泡巨噬细胞的损伤刺激作用，并通过基质金属蛋白酶-9（MMP-9）及单核细胞趋化蛋白-1（MCP-1）的表达程度来分析其诱发气道炎症进而引发肺大疱破裂形成自发性气胸的相关性。通过分析标本中肺泡巨噬细胞、基质金属蛋白酶-9、单核细胞趋化蛋白-1、肺弹力纤维的表达变化特点，探讨其对自发性气胸肺大疱形成和演变的影响。进而可以对该病的发生机制有明确的认识，进一步为结合临床防治该病提供基础理论依据。  

Objectives of Study：

Through the detection and observation of clinical specimens, this paper analyzes and expounds the injury and stimulation effect of air fine particle deposition on alveolar macrophages, and analyzes the correlation between the induction of airway inflammation and the rupture of pulmonary bullae to form spontaneous pneumothorax through the expression of matrix metalloproteinase-9 (MMP-9) and monocyte chemoattractant protein-1 (MCP-1).By analyzing the expression characteristics of alveolar macrophages, matrix metalloproteinase-9, monocyte chemoattractant protein-1 and pulmonary elastic fibers, to explore their effects on the formation and evolution of pulmonary bullae in spontaneous pneumothorax. Then we can have a clear understanding of the pathogenesis of the disease, and further provide a basic theoretical basis for clinical prevention and treatment of the disease.

纳入标准：

（1）病例组气胸受试者纳入标准：①年龄≦30岁；②符合中华医学会的《临床诊疗指南-胸外科分册》中的相关诊断标准,且均需接受胸腔镜手术治疗的患者；③无麻醉、手术等禁忌；④KPS评分>80分，⑤术前无高血压、糖尿病、冠心病或高血脂等慢性疾病；⑥术前无肝硬化或其他影响凝血功能的疾病；⑦术前心肺功能良好，代偿能力较强；⑧知情同意。
（2）对照组肺良性病变受试者纳入标准：①年龄≦60岁；②符合中华医学会的《临床诊疗指南-胸外科分册》中的相关诊断标准，且均接受手术治疗的患者；③无麻醉、手术等禁忌；④KPS评分>80分，⑤术前无高血压、糖尿病、冠心病或高血脂等慢性疾病；⑥术前无肝硬化或其他影响凝血功能的疾病；⑦术前心肺功能良好，代偿能力较强；⑧知情同意。

Inclusion criteria

1. Inclusion criteria of pneumothorax subjects in case group:
(1) Aged <= 30 years;
(2) Patients who meet the relevant diagnostic criteria in the clinical diagnosis and treatment guidelines - thoracic surgery volume of the Chinese Medical Association and need thoracoscopic surgery;
(3) No contraindications such as anesthesia and operation;
(4) KPS score > 80 points.
(5) There were no chronic diseases such as hypertension, diabetes, coronary heart disease or hyperlipemia before operation.
(6) No liver cirrhosis or other diseases affecting coagulation function before operation;
(7) Preoperative cardiopulmonary function was good and compensatory ability was strong;
(8) Informed consent.
2. Inclusion criteria of subjects with benign lung lesions in the control group:
(1) Aged <= 60 years;
(2) Patients who meet the relevant diagnostic criteria in the clinical diagnosis and treatment guidelines - thoracic surgery volume of the Chinese Medical Association and receive surgical treatment;
(3) No contraindications such as anesthesia and operation;
(4) KPS score >80 points.
(5) There were no chronic diseases such as hypertension, diabetes, coronary heart disease or hyperlipemia before operation.
(6) No liver cirrhosis or other diseases affecting coagulation function before operation;
(7) Preoperative cardiopulmonary function was good and compensatory ability was strong;
(8) Informed consent.

排除标准：

①年龄超过60岁，KPS评分<60分；②合并严重的心肺疾病患者；③血常规、肝肾功能、凝血功能异常，存在手术禁忌证；④病情出现了预料外变化；⑤既往有精神病史,无法配合研究。

Exclusion criteria：

① Over 60 years old, KPS score < 60; ② Patients with severe cardiopulmonary disease; ③ Abnormal blood routine, liver and kidney function and coagulation function, with surgical contraindications; ④ Unexpected changes in the condition; ⑤ Patient had a history of mental illness and was unable to cooperate with the study.
