研究目的：

(1)超声观测拔管后膈肌运动幅度、膈肌厚度变化率及恢复率与术后患者氧合指标的相关性； (2)评估运用膈肌超声改善全身麻醉拔管后患者术后氧合功能的可行性;  

Objectives of Study：

(1) The correlation between postoperative oxygenation index and diaphragm motion amplitude, change rate of diaphragm thickness and recovery rate after extubation was observed by ultrasound; (2) To evaluate the feasibility of using diaphragmatic ultrasound to improve postoperative oxygenation function of patients after extubation under general anesthesia;

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

患者性别不限，ASAⅡ-Ⅲ级，年龄大于65岁，行Trendelenburg体位的手术 ，所有患者术前均神志清晰,相关检验检查基本正常；

Inclusion criteria

There was no limitation in gender, ASA ⅱ - ⅲ, age > 65 years old, and Trendelenburg position surgery was performed. All patients were conscious before surgery, and relevant examinations were basically normal

排除标准：

既往有呼吸系统疾病史，既往心肺及胸部手术病史，胸廓畸形，胸腹腔积液或胸腔巨大肿瘤等;合并严重心血管疾病的患者，如血压大于180/110mmHg，三个月心绞痛及心梗患者；肝肾功能不全和精神疾病，神经肌肉病史，恶性高热家族史，视力、听力障碍无法配合的患者；语言沟通障碍以及拒绝参与实验患者。

Exclusion criteria：

Previous history of respiratory disease, previous history of cardiopulmonary and chest surgery, thoracic malformation, pleural and abdominal effusion or large thoracic tumor; Patients with serious cardiovascular disease, such as blood pressure greater than 180/110 MMHG, angina pectoris and myocardial infarction at 3 months; Patients with liver and kidney dysfunction and mental illness, neuromuscular history, family history of malignant high fever, vision and hearing impairment; Verbal communication problems and refusal to participate in the experiment.
