研究目的：

对于已预料的明确困难气道，学者们推荐清醒镇静表面麻醉下实施气管插管。目前用于清醒镇静表面麻醉下行气管插管的麻醉药各有优缺点，还没有一种各方面都很理想的麻醉药。瑞马唑仑是一种新型超短效的苯二氮草类镇静药，为我国1.1类新药，具有起效快、维持和恢复时间短、无蓄积、代谢不依赖肝肾功能、对呼吸循环影响小、无严重副作用、能够被氟马西尼逆转等特点，极有可能成为理想的麻醉药。国内外的临床研究显示，瑞马唑仑用于无痛诊疗的镇静优势明显突出。但是目前尚无资料报道瑞马唑仑用于困难气道患者清醒镇静表面麻醉下行气管插管的麻醉效果。本课题拟先将瑞马唑仑用于可疑困难气道患者中，探索出瑞马唑仑用于清醒镇静表面麻醉下行气管插管的合适剂量，再进一步探讨瑞马唑仑用于困难气道患者清醒镇静表面麻醉下行气管插管是否安全、有效，为临床工作提供参考依据。  

Objectives of Study：

For the clearly expected difficult airway, scholars recommend tracheal intubation under mild sedation or topical anesthesia. At present, the anesthetics used for mild sedation and topical anesthesia for tracheal intubation have their own advantages and disadvantages, there is no anesthetic that is ideal in all aspects. . Remimazolam is a new type of ultra-short-acting benzodiazepine sedative. It is a class 1.1 new drug in our country.It has fast onset, short maintenance and recovery time, no accumulation, metabolism independent of liver and kidney function, and has a slight impact on circulatory and respiratory function. It has the characteristics of no serious side effectsand can be antagonized by flumazenil, which make it likely to be an ideal anesthetic. Clinical studies at home and abroad have shown that the sedation advantage of remimazolam for painless diagnosis and treatment is obvious. However, there is no data to report the anesthesia effect of remimazolam on tracheal intubation of patients with difficult airway under reserved respiratory sedation and local anesthesia. This project intends to use remimazolam in patients with suspected difficult airway, to explore the appropriate dose of remimazolam for tracheal intubation under retained respiratory sedation and local anesthesia, and to further explore the safety and effectiveness of remimazolam for tracheal intubation under retained respiratory sedation and local anesthesia, so as to provide reference for clinical work.

纳入标准：

第一部分试验纳入标准：ASAI～Ⅱ级，年龄18～65岁，体质量指数（BMI）≤30 ㎏/m2。可疑困难气道患者：张口度≤2 cm，预计无面罩通气困难。
第二部分试验纳入标准：ASAI～Ⅱ级，年龄18～65岁，体质量指数（BMI）≤30 ㎏/m2，已明确的困难气道患者。

Inclusion criteria

The first part of the test inclusion criteria: ASAI-Ⅱ, age 1865 years old, body mass index (BMI) ≤30 ㎏/m2. Suspected difficult airway patients: mouth opening ≤2 cm, no mask ventilatory difficulties are expected.
The second part of the trial inclusion criteria: ASAI-Ⅱ grade, 1865 years old, body mass index (BMI) ≤30 ㎏/m2, patients with clear difficult airway.

排除标准：

第一部分试验排除标准：预计有面罩通气困难患者；术前有长期服用镇静药史或吸毒史患者；术前存在严重心、肺、肝、肾疾病者或血常规、血生化指标明显异常者；对麻醉药过敏者。
第二部分试验排除标准：术前有长期服用镇静药史或吸毒史患者；术前存在严重心、肺、肝、肾疾病者或血常规、血生化指标明显异常者；对麻醉药过敏者。

Exclusion criteria：
