研究目的：

本研究通过集束化护理策略优化重症脑卒中患者压疮的预防及护理流程，完善压疮治疗措施，降低压疮发生率，提高压疮治愈率，提高患者生活质量，提高患者舒适度与满意度。  

Objectives of Study：

The cluster nursing strategy is adopted to optimize the prevention and nursing process of pressure ulcers in patients with severe stroke, improve the treatment measures of pressure ulcers, reduce the incidence of pressure ulcers, improve the cure rate of pressure ulcers, improve the quality of life of patients, and improve the comfort and satisfaction of patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）符合各类脑卒中诊断标准，并经头颅CT扫描或MRI确诊；
2）住院时间≥24 h；
3）入住重症监护病房，符合急性生理学和慢性健康状况（APACHE）Ⅱ评分>16 分，格拉斯哥昏迷评分法（GCS）评分≤12 分；
4）自愿参与并签署知情同意书。

Inclusion criteria

1) Meet various diagnostic criteria for stroke and be confirmed by cranial CT scan or MRI;
2) Duration of hospital stay >=24 h;
3) Adults in the intensive care unit with acute physiology and chronic health status (APACHE) II>16 points, Glasgow coma score (GCS) <=12 points;
4) Voluntarily participate and sign the informed consent.

排除标准：

1）患有皮肤病（如系统性红斑狼疮、银屑病、癣等）以及皮肤损伤（如烧伤、破损、溃疡等）的患者；
2）患者的病情不允许翻身者（如骨盆骨折、颈椎骨折、强迫体位等）；
3）患者皮肤肤色较深，难以进行皮肤状况的评估者；
4）研究期间中途出院、转科、死亡或各种原因无法继续参加本试验的患者。

Exclusion criteria：

1) Patients suffering from skin diseases (such as systemic lupus erythematosus, psoriasis, ringworm, etc.) and skin lesions (such as burns, breakages, ulcers, etc.);
2) The patient's condition is not allowed to turn over (such as pelvic fracture, cervical fracture, forced position, etc.);
3) It is difficult to assess the skin condition due to the dark skin tone of the patient;
4) Patients who were discharged from the hospital, transferred to another department, died or could not continue to participate in the study for various reasons during the study period.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1、本研究所需所有压力性损伤评定由本专业同一位不参与压力性损伤治疗的具有中级职称的老师进行。 2、数据录入，分别由2个人进行2次数据录入，再比对其正确率，如有不相同之处，查找原始数据进行修改。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. This study requires that all stress injury assessments should be conducted by the same teacher with intermediate professional title who is not involved in stress injury treatment. 2. Data entry: data entry was performed by two people for two times, and then the accuracy was compared. If there was any difference, the original data was searched for modification.
