Objectives of Study：

Objectives of Study: 1. To evaluate the effect of the community-family-mother-child (CFMC) on the indicators of childhood growth and development, including body proportionality (height / weight2, or BMI), and adiposity in boys and girls at 5 years old; 2. To evaluate the effect of the CFMC programme on family and child lifestyles (nutrition, physical activity and habits); including differences between boys and girls; 3. To evaluate the effect of the CFMC intervention on early indicators of the bodys processing of nutriments such as glucose (metabolism) in boys and girls from birth to 5 years old; 4. Through the creation of the data and biobank repository, the team will be able to identify biomarkers that are predictive of longer term health of the child, to study underlying mechanisms, and to evaluate the impacts of the intervention; 5. To evaluate the feasibility of implementing the CFMC intervention as a sustainable part of the approach to childhood health and lifestyle (in hospitals, Maternal Child Health (MCHs) units and childcare facilities to increase healthy lifestyle at the community level).

从孕前（1800）和孕早期（2700）招募妇女共计4500人，随访贯穿这个孕期，分娩时期，于分娩后6周，3个月，6个月，1,2,3,5岁对母婴随访调查。为了涵盖生活习惯和行为的改变，丈夫也将被招募入组。我们将收集所有参与研究者的生物样本以建立生物样本库。 这一研究主要包括两部分：对照组（常规保健），干预组（常规保健和干预研究）。除常规保健外，对照组也将下载软件，我们将提供关于母婴安全和儿童保健的信息。 除了常规保健外，干预组的妇女将： 1. 以研究对象为中心，一对一，面对面、多种生活方式频繁的访谈； 2. 获取额外的小组教育和社会支持； 3. 为妇女提供个体化的达成行为改变目标和克服障碍的相关信息； 4. 研究对象生活方式（如运动和饮食）为目标的自我监测软件，也将提供关于儿童健康和安全的信息; 5. 以社区为基础的活动。 干预的目的主要是为了促进积极地行为改变，主要包括：营养、运动、吸烟、压力和身体健康、睡眠质量和母乳喂养等方面。 

Description for medicine or protocol of treatment in detail：

Women will be recruited and followed from the preconception period (N=1800) or from beginning of pregnancy (N=2700), throughout pregnancy (1st, 2nd, 3rd trimester, and delivery). Mother\child dyads will be followed at 6 weeks, 3 months, 6 months and 1, 2, 3, 5 years post-delivery. Partners will also be recruited (facultative) in order to be involved in the lifestyle and behavioral changes. Bio specimens will be collected for the biobank for all the participants. The study includes two arms: control group (standard care) and intervention group (standard care & research intervention). In addition to the standard care, the control group will have access to web-based tools and Apps that provide information on child health and safety. In addition to standard care, women in the intervention group (Healthy conversations group) will undergo: 1. Patient centered, one-on-one, face-to-face sessions, and frequent contacts with a multidisciplinary team; 2. Access to additional group educational activities and social support; 3. Text Messaging that is tailored to the individual woman's personal goal and barriers to behaviour change; 4. Motivational web-based tools and Apps for self-monitoring of the participant lifestyle goals (Physical activity, diet) and also to provide information on child health and safety. 5. Community based activities The intervention aims to positively change behaviour regarding a) nutrition, b) physical activity, c) smoking, d) stress and physical health, e) sleep quality and f) breastfeeding.

计划六个月内怀孕；

Inclusion criteria

Clusters (units of randomization) criteria:
36 maternal child health units associated with one of six participating maternity hospitals.
Patient criteria:
Women between 20 – 42 years of age;
Speaks Mandarin;
Either a registered Shanghai resident, married to a registered Shanghai resident, or non-resident but has lived in Shanghai for 5 years;
Able to read, understand and sign the consent form.
Plans to deliver in a participating study hospital;
MCHs unit where the child will be registered has to be identified and must be a participating SCHeLTI MCH.
Inclusion criteria specific to the preconception period:
Planning pregnancy in the next 6 months.
Inclusion criteria specific to the pregnancy period:
Singleton pregnancy;
Less than 14 weeks of gestation at the time of enrolment.
Fetal viability confirmed by ultrasound (positive fetal heart)

HIV positivity, measureable viral load of hepatitis B or C, hepatitis B core or e-antigen positive, Hepatitis C core antigen positive.
Women with known major heart conditions leading to functional limitations or deficit, thalassemia major, cystic fibrosis, pre-existing type 1 diabetes mellitus, type 2 diabetes mellitus and/or chronic hypertension dependant on medication;
Drug abuse;
Planning to move out of the district within 5 years.
Exclusion criteria specific to preconception period:
Planning to conceive by in vitro fertilization.
Exclusion criteria specific to pregnancy period:
Known multiple pregnancy;
Known major fetal malformation;
"in vitro fertilization" pregnancy
