本研究旨在通过对阑尾炎聚集性的研究，证明“急性出血性阑尾炎”是一种新型的非穿孔性阑尾炎，在不同省份发生，具有传染病的流行病学特征。第二个目的是确定阑尾炎聚集性发生的环境，并为发现新的阑尾炎聚集性和证明新的阑尾炎实体提供建议。  

Objectives of Study：

The aim of this study was to demonstrate through studying cluster of appendicitis that “acute hemorrhagic appendicitis” was a new entity of non-perforated appendicitis occurring reproducibly in different provinces, with features of infectious diseases in epidemiology. A second aim was to confirm common settings of cluster of appendicitis and to provide suggestion to find new cluster of appendicitis and demonstrate new entity of appendicitis.

药物成份或治疗方案详述：

这是一项关于阑尾炎聚集性的流行病学研究和临床病例分析，不属于治疗研究。因此，有关干预措施部分我们不能填写具体内容，用填“n/a”代替。本研究绝大部分是回顾性的，仅健康教育是前瞻性的，所以我们填写注册号状态：回顾性研究。 根据美国德克萨斯州、中国武汉等地阑尾炎聚集性发生的情况，我们认为集体生活单位可能是阑尾炎聚集性的多发地方，因此我们关注学校和军营。我们对阑尾炎聚集性进行现场调查，并对流行病学、临床、实验室和病理资料进行分析。 按照Reingold AL的[25]方法对阑尾炎的聚集性进行流行病学调查。 流行病学、临床和病理研究 在流行病学调查中，我们将按人群、时间、地点收集阑尾炎发病率、人群、环境、发病时间和持续时间、暴露史、生活习惯、环境卫生以及我们采取的预防措施等资料。 临床研究采用标准化数据采集表收集患者临床、实验室检查信息，包括临床表现、体格检查、体温、白细胞计数、中性粒细胞百分比。 病理检查，穿孔性阑尾炎或阑尾脓肿合并为单一的类别称为“穿孔性阑尾炎”。非穿孔性阑尾炎定义为除穿孔性阑尾炎或阑尾脓肿外的任何阑尾炎。 我们将仔细检查阑尾炎群集的病理特征。如前所述，由于聚集性往往由共同的原因导致，因此聚集性病例中几乎每一位患者都属于同一阑尾炎实体，并出现在同一疾病的某一阶段。结合每个患者的病理特征，我们可以了解阑尾炎聚集病例的全部自然史。这样我们就可以证明穿孔性阑尾炎和非穿孔性阑尾炎是否为不同的实体或同一实体阑尾炎的不同阶段。 本研究经武汉大学医学院医学伦理委员会批准，并获得每个受访者的书面知情同意。 纳入本研究需要符合以下纳入标准:急性阑尾炎的典型临床表现和急性阑尾炎的组织学特征。 

Description for medicine or protocol of treatment in detail：

According to the setting of cluster of appendicitis in Texas, USA and Wuhan, China, we assumed that collective living units might be places where cluster of appendicitis occurs frequently, so we paid attention to school. military camp. We studied the cluster of appendicitis by means of field investigation and analyze epidemiologic, clinical, laboratory and pathologic data. Epidemiological investigation of cluster was carried out according to Reingold AL’s methods[25]. Epidemiological, Clinical and Pathological Investigation For epidemiological investigation, we will collect data of distribution by person, time and place, including incidence rate of appendicitis, population, setting, start time and duration, exposure history, living habits, environmental sanitation, and preventive measure we take. For clinical investigation, the patients’ clinical, laboratory examination information were collected on a standardized data collection form, including clinical manifestation, physical examination, body temperature, white blood cell count and percentage of neutrophile. For pathological investigation, perforated appendicitis or appendiceal abscess were aggregated into a single category called “perforated appendicitis”. Nonperforated appendicitis was defined as having any appendicitis except for perforated appendicitis or appendiceal abscess[6]. We will examinate pathological feature of appendicitis in cluster carefully. As stated before, because cluster often results from common cause, therefore almost every patient in cluster belongs to the same entity of appendicitis and appeared in certain stage of same entity. Connecting each patient’s pathological feature, we can know full natural histories of appendicitis in cluster. In this way, we can demonstrate whether or not perforated appendicitis and non-perforated appendicitis are different entities or different stage of same entity of appendicitis. The study was approved by ethic board at collaborating hospital and Wuhan University School of Medicine, written informed consent will be obtained from each respondents. To be included in this study, the following inclusion criteria have to be met: typical clinical manifestations of acute appendicitis and histologic features of acute appendicitis.  

纳入标准：

纳入本研究需要符合以下纳入标准:急性阑尾炎的典型临床表现和急性阑尾炎的组织学特征。 所有阑尾切除术的病理切片均在武汉大学中南医院病理科复查。

Inclusion criteria

To be included in this study, the following inclusion criteria have to be met: typical clinical manifestations of acute appendicitis and histologic features of acute appendicitis. All histologic slides from appendectomy were re-examined in Department of Pathology, Zhongnan Hospital of Wuhan University.

病历记录不全和到非合作医院治疗的病人予以排除。

Exclusion criteria：

Patients with incomplete medical records and treatment at non-cooperative hospitals were excluded.
