研究目的:

儿童龋病对学龄前儿童的身心发育造成了严重的不良影响。目前学界认为龋病与口腔细菌紊乱有着密切关系。因此有必要比较患病与健康儿童在口腔细菌方面的差异。在此基础上,再根据患病口腔菌群特点,开发一种能够有效防治儿童龋病的新方法是十分重要的。益生菌是一类能够对人体产生有益作用的细菌,能够有效拮抗致病菌生物膜,国外已有多项临床研究表明益生菌可有效降低降低儿童口腔中的致病菌含量并且是安全有效的。本实验室在前期研究基础上,发现植物乳杆菌CCFM8724可有效缓解动物龋病模型,但其临床效果是未知的。因此我们开展了本研究。  

Objectives of Study:

Children with caries have a serious adverse effect on the physical and mental development of preschool children.At present, it is believed that dental caries is closely related to oral bacterial disorders, so it is necessary to compare the differences in oral bacteria between sick and healthy children.On this basis, it is very important to develop a new method to effectively prevent and treat children caries according to the characteristics of oral microbiota.Probiotics are kinds of bacteria that can have beneficial effects on the human body and can effectively antagonize the biofilm of pathogenic bacteria. A number of clinical studies abroad have shown that probiotics can effectively reduce the content of pathogenic bacteria in children's oral cavity and are safe and effective.On the basis of our previous study, we found that Lactobacillus plantarum CCFM8724 can effectively alleviate caries in animal models, but its clinical effect is unknown.So we conducted this study.

药物成份或治疗方案详述:

本实验采用随机双盲实验,共招募100名重度龋病儿童(3-6岁),每组50人,分为益生菌组和安慰剂组。益生菌组服用的是植物乳杆菌CCFM8724,辅料为麦芽糊精。安慰剂组服用的是麦芽糊精。植物乳杆菌CCFM8724来源于健康人体,被认为具有降低口腔内致病菌的含量,麦芽糊精被认为没有任何效果。嘱咐病人在0,14,28,42天内来院复诊并采集样品(包括唾液和龈上菌斑)。其中唾液样品利用qPCR技术,测量游离致病菌的含量,牙菌斑样品利用16S rDNA技术,测量益生菌服用前后,对口腔菌群的影响。 

Description for medicine or protocol of treatment in detail:

A total of 100 children (3-6 years old) with severe caries were enrolled in this randomized, double-blind trial, with 50 children in each group. They were divided into probiotics group and placebo group.The probiotics group took Lactobacillus plantarum CCFM8724 with the excipient of maltodextrin.The placebo group received maltodextrin.Lactobacillus plantarum CCFM8724, derived from healthy humans, is thought to reduce the level of oral pathogens, while maltodextrin is thought to have no effect.Inform the patient to return to the hospital within days 0, 14, 28, and 42 to collect samples (including saliva and supragingival plaque).The content of free pathogenic bacteria in saliva samples was measured by qPCR, and the influence of probiotics on oral flora was measured by 16S rDNA technology in dental plaque samples before and after probiotics administration. 

纳入标准:

①实验过程能遵守相关规则,年龄:3~6岁
②身体健康,无严重慢性病或过敏史
③健康无龋儿童的DMFT=0, 龋齿儿童的DMFT≥6
④乳牙已全部萌出,并能配合检查
⑤研究期间的生活方式保持不变

Inclusion criteria

① The experimental process can abide by the relevant rules, age: 3~6 years old
② Good health, no history of serious chronic disease or allergy
③ The DMFT of healthy children without caries was 0, and the DMFT of caries was ≥6
④ The baby teeth have all sprung out, and can cooperate with the examination
⑤ Lifestyle remained unchanged during the study period

排除标准:

①患有全身系统性疾病(如糖尿病,肾病,自身免疫性疾病)
②4周内接受氟化物治疗
③4周内接受抗生素治疗
④6个月内接受激素和免疫抑制剂治疗
⑤口腔内出现感染病灶,牙龈炎,根尖周炎,口腔溃疡,急性咽炎,急性扁桃体炎等口腔疾病
⑥研究期间使用其他益生菌产品(包括益生菌片剂,益生菌冲剂,益生菌口服液,益生菌酸奶,益生菌牙膏等)或木糖醇口香糖。
⑦试验期间如因其他疾病需要使用抗生素的儿童

Exclusion criteria:

① Have systemic diseases (such as diabetes, kidney disease, autoimmune disease)
② Fluoride treatment was given within 4 weeks
③ Antibiotic treatment within 4 weeks
④ Hormone and immunosuppressive therapy were received within 6 months
⑤ Oral infection lesions, gingivitis, periapical periodontitis, oral ulcers, acute pharyngitis, acute tonsillitis and other oral diseases
⑥ Other probiotic products (including probiotic tablets, probiotic granules, probiotic oral solution, probiotic yogurt, probiotic toothpaste, etc.) or xylitol chewing gum were used during the study period.
⑦ Children who need antibiotics for other diseases during the trial

在比较患病与健康儿童口腔菌群差异方面,由富有经验的口腔科主任依据国际龋病检测与评估系统(ICDAS-II)标准,选择健康或龋病儿童。在植物乳杆菌CCFM8724防治儿童龋病方面,由申请注册人按照随机数字表法,将患者随机分为益生菌组或安慰剂组。安慰剂组与益生菌组的区别仅在于是否添加了益生菌。其他方面,如包装,外观,气味等方面均无区别。由医生将益生菌或安慰剂交给患病儿童家长,医生和患者家属不知道自己分的是益生菌或是安慰剂。待全部实验结束后,由申请注册人公布具体分组情况。课题负责人负责整个过程的进展及监督。
