研究目的：

原发免疫性血小板减少症是一种复杂的，多种免疫机制共同参与的获得性自身免疫性出血性疾病，至今发病机制不明。近年来的研究认为, T细胞免疫异常在其发病中起到一定的作用，T细胞的活化需要协同刺激信号，来影响T 细胞的活化增殖或凋亡。我们在前期实验的研究中发现，共刺激分子的表达可能与ITP的发病有关。而负性共刺激途径程序性死亡分子PD-1及其配体PD-L1与多种免疫性疾病的免疫调节有关。本项目研究旨在通过检测ITP患者发病期及治疗后PD-1/PD-L1在T细胞上的表达及T细胞分泌的相关细胞因子的动态变化，探讨 PD-1/ PD-L1信号通路对ITP患者T细胞增殖的调控作用及其对细胞因子分泌的影响，来阐明该通路在ITP发病机制中所起的作用，从而为ITP的免疫靶向治疗提供新思路。  

Objectives of Study：

Primary immune thrombocytopenia (ITP) is a complicated, acquired, autoimmune hemorrhagic disease with many immune dysfunctions. The etiology of ITP remains unclear by now. Recent studies found that T-cell immune abnormalities played a role in the pathogenesis of ITP. T-cell activation require co-stimulatory signals which determine the activation、proliferation or apoptosis of T cell. Our previous study found that the expression of co-stimulatory factor may be related to ITP. Recently，programmed cell death molecule PD-1 and its ligand PD-L1 in the negative co-stimulatory pathways were discovered in immunology. This pair of molecules showed potential values in pathogenesis and treatment of this disease. The aim of the study is to explore the effect of PD-1 / PD- L1 signaling pathway in T cell proliferation, to elucidate its role in the pathogenesis of ITP and to provide theoretical basis for immune targeting prevention in ITP through researching the dynamic changes of PD-1/PD-L1 signaling pathway T cells and related cytokines in patients with ITP before and after treatment.

纳入标准：

入选病例标准如下：1.符合ITP诊断标准:①多次实验室检查血小板计数减少；②脾脏不肿大；③骨髓检查巨核细胞数增多或正常，有成熟障碍；④除外其它继发性血小板减少症;2.均未接受过血小板输注和脾切除术等治疗;3.患者未使用糖皮质激素、免疫抑制剂或已停用1个月以上。4.年龄18-75岁，性别不限;5.自愿接受临床研究，并签署知情同意书。
入选正常对照标准如下：均为健康体检者，无发热及近期服用药物史，无明显的心、肺、肝、肾等功能障碍,家属及本人知情同意并签署知情同意书。

Inclusion criteria

The case inclusion criteria are as follows:1.Conform to the diagnostic criteria of ITP: ① decreased platelet counts in many laboratory tests; ② Without splenmegaly; ③ Elevated or normal number of megakaryocytes in the bone morrow examination with dysmaturity; ④ With exception of other secondary thrombocytopenic purpura;2.Without received platelet transfusions and splenectomy treatment;3.Without used of corticosteroids, immunosuppressants or disabled a month or more;4.Age 18 to 75 years old,male or female;5.volunteer to participate in clinical trial, and signed informed consent.The healthy contorl inclusion criteria are as follows:Healthy volunteers;Without fever and history of drug taking recently,Without ditinct dysfunction of heart, lung, liver, kidney and so on;The Families and volunteers Informed and signed informed consent.

排除标准：

排除标准如下：继发性血小板减少性疾病（如：再生障碍性贫血、急性白血病、骨髓增生异常综合征、急性造血功能停滞等）;近1个月输注血小板史及服用免疫抑制剂。

Exclusion criteria：

The case exclusion criteria are as follows:With exception of other secondary thrombocytopenic purpura(eg:Aplastic anemia, Acute Leukemia, Myelodysplastic syndrome, Acute hematopoietic function stagnation and so on); Platelet transfusion history and taking immunosuppressant nearly a month
