研究目的：

（1）采取经门诊有序安排相关检查和详细病史回顾的方法，了解在学会委员所在三级以上医院老年糖尿病管理现状。（2）通过多中心、随机分组、开放性、平行对照为期56周的前瞻性临床研究，观察在血糖控制欠佳（HbA1c≥7.5%）的中国老年（≥60岁）2型糖尿病患者中，规范化降糖策略和综合管理的有效性和安全性（包括心血管安全）。  

Objectives of Study：

(1) To investigate the management status of elderly patients with type 2 diabetes among several hospitals in China through physical examination in clinics and chart review; (2) To assess the effectiveness and safety of the method of optimal diabetic management in Chinese elderly patients (aged ≥ 60 years) who had an inadequate blood glucose control (the level of hemoglobin A1C (HbA1c) higher than 7.5%) in a multicenter, randomized, parallel controlled clinical trial for 56 weeks.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）已确诊的老年（≥60岁）2型糖尿病（WHO 1999年标准）患者；
（2）基线调查时HbA1c≥7.5%；
（3）自愿签署知情同意书，有能力参加研究，有经济条件支付研究必须治疗费用。

Inclusion criteria

(1) Diagnosed type 2 diabetes with age ≥ 60 years;
(2) HbA1c ≥ 7.5% at baseline;
(3) Given informed consent and have the ability to afford treatment cost.

排除标准：

（1）年龄<60岁或>90岁；
（2）1型糖尿病；
（3）HbA1c < 7.5%或HbA1c > 14%，和/或空腹血浆葡萄糖> 250 mg/dL (> 13.9 mmol/L)；
（4）半年内有严重心脑血管事件发生，存在尚未治愈且影响预后的疾病（活动性或非活动性恶性肿瘤，CKDIV期以上患者，慢性心衰，慢性呼衰，肝硬化中晚期，神经、精神、血液、皮肤疾病…）；其它重要的全身疾病（例如严重的感染和外伤、营养不良、脱水、重大外科手术、具有显著临床意义的低血压和低氧情况)，其它导致低氧血症的疾病；
（5）患者的预期寿命短；或者研究者认为有任何可能干扰研究药物评价的医学情况；
（6）有无感知的低血糖发作病史；
（7）有血红蛋白病或溶血性贫血病史，筛选前3个月内曾输注过血制品或血浆制品；
（8）筛选前6个月内有药物或酒精滥用史；
（9）筛选时，实验室检查发现以下结果，或者根据研究者判断有其它具有显著临床意思的实验室异常，可能妨碍本研究的安全完成或者限制疗效评价：
○1女性：血红蛋白< 10 g/dL，男性：血红蛋白< 11 g/dL，和/或中性白细胞<1,500/mm3，和/或血小板< 100,000/mm3；
○2ALT、AST或总胆红素≥正常范围上限3倍；
○3血清肌酐≥正常范围上限2倍，或eGFR<30 ml/min；
（10）患者处于不能理解本研究的性质、范围和可能出现后果的精神状态，包括血糖监测要求、有证据证实的不合作态度和/或无法回来接受随访访视，不可能完成研究或不能遵从之后的医学指导，缺乏完成研究的能力（智能、体能障碍），依从性差。
（11）与研究药物有关的排除标准： 有中国药物标签所注任何的禁忌症或者已知对研究药物任何成分过敏的患者。

Exclusion criteria：

(1) Age < 60 years or > 90 years;
(2) Type 1 diabetes;
(3) HbA1c <7.5% or >14%, and/or fasting plasma glucose > 250 mg/dL (> 13.9 mmol/L);
(4) Had cardiovascular and cerebrovascular events within half a year, had uncured diseases with worst prognosis (e.g. active or inactive malignant tumor, chronic kidney disease stage IV or above, chronic heart failure, chronic respiratory failure, middle-later stage hepatocirrhosis, neurological disease, mental disease, hematologic disorders, dermatogic disease), other systemic diseases (e.g. serious infection or trauma, malnutrition, dehydration, major surgery, serious hypotension or hypoxemia);
(5) Patients with a short life expectancy, or have a medical status that may affect the effectiveness of medicine;
(6) Had a history of unintentional hypoglycemic episodes.
(7) Had a history of hemoglobin disorders or hemolytic anemia, or had a history of blood transfusion within 3 months.
(8) Had a history of drug or alcohol abuse within 6 months.
(9) Hemoglobin < 10 g/dL (female) or 11 g/dL (male), and/or neutrophil < 1,500/mm3, and/or platelet < 100,000/mm3.
(10) Patients who do not understand the aim and requirement of the study, who do not accept regular follow-up, who do not comply with the optimal management, and who lack the ability of finishing study.
(11) Had any contraindications of taking antidiabetic drugs, or had allergic reaction to these drugs.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本研究的原始资料均以手机照相形式存入研究专用移动硬盘保存。研究数据通过病例报告表记录后，经本研究专用数据录入程序转录入本研究专用数据库（浙江大学合作建制），录入数据均由2人核对，并经形式审查。有专门数据管理小组进行数据收集核准和管理。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

The photo of all data are taken by phones, and stored in a dedicated mobile hard disk. The data is recorded by case report form and a special-purpose database (developed by Zhejiang University) is the main repository to store the data. All records are double checked up and managed by a specially-assigned group.
