研究目的：

本课题旨在制定适用于中国人群，信度和效度较高的糖尿病足部护理评估表，调查2型糖尿病足高危患者，实施3种不同方式的措施前和干预后24周内，足部护理行为和足部基本状况，从而更好地指导糖尿病健康教育工作，选择最佳的时机和健康教育方法，提高糖尿病足高危患者的足部护理行为，减少危险因素暴露，降低溃疡率和截肢率，提高生活质量。  

Objectives of Study：

The purpose is to develop a scale with good reliability and validity, which is suitable for Chinese population and to investigate the condition of foot-care behavior and basic status of feet of high risk in type 2 diabetic foot patients before and after the intervention in twenty-four weeks. Therefore, it can guide the future clinical health work by selecting the best timing and methods to increase foot care behaviour and decrease risk factors exposed, then reduce the occurrence rate of diabetic ulcer and amputation and improve the quality of life.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合WHO 1999年糖尿病的诊断和分型标准，年龄为18-74岁的2型糖尿病患者；②糖尿病足高危人群是根据国际糖尿病足组制定的糖尿病足危险因素分类系统，合并有1种或多种危险因素的患者，具体包括：感觉性神经病变和（或）足畸形、或骨的突起和（或）周围血管病变，和（或）既往溃疡史/截肢史；③改良的Barthel指数评分≥60分者；④有手机等便利通讯设备，便于出院后电话随访；⑤思维正常，能进行语言沟通者；⑥经患者和家属同意，愿意加入本研究者。

Inclusion criteria

① corresponding the diabetic diagnosis and classification of WHO in 1999, which is from 18 to 74 year old type 2 diabetes patients; ② the high risk population of diabetic foot is according to the international diabetic foot group's diabetic foot risk classification system with one or more risk factors, including;sensory neuropathy and(or) foot deformity, or bone prominence and/or peripheral vascular disease, and (or) history of ulcer history/amputation; ③ the score of improved the Barthel index is sixty or more points; ④ having convenient communication device such as mobile phone to facilitate telephone follow-up after discharge; ⑤ someone whose is normal thinking and language communicating; ⑥ with the consent of patient and family, who is willing to join the research.

排除标准：

①合并急性或其他严重的糖尿病相关并发症，如酮症、严重的视网膜病变、糖尿病足溃疡；②严重的认知功能障碍者；③严重肝脏疾病；④合并大腿截肢术、骨折、肌无力或下肢血管外科手术后等严重影响躯体功能，生活完全不能自理者；⑤带状疱疹、肿瘤等病因引起的周围神经病变者；⑥静脉曲张者；⑦合并抑郁等精神疾病者；⑧既往6个月内参加与糖尿病足护理相关的健康教育课程者；⑨单眼最差矫正视力＜0.3。

Exclusion criteria：

① complicating acute or other serious complications related to diabetes, such as ketosis, severe retinopathy, diabetic foot ulcer; ② with severe cognitive impairment; ③ serious liver disease; ④ complicating leg amputation, fracture, muscle weakness or the lower extremities vascular surgery which seriously affects physical function to care himself completely; ⑤ peripheral neuropathy which is caused by shingles, cancer or other causes; ⑥ Varicose veins; ⑦ complicating depression or some other mental disorders; ⑧ participated in health education courses related to diabetic foot nursing in six months; ⑨ the worst pet-name ruby monocular vision correction less than 0.3.
