研究目的：

To demonstrate that diabetes educator (DE) working with healthcare providers can improve the blood glucose levels, blood pressure, lipid profile and to report that how they enhance the health related of life (HRQol)and Medication adherence

To demonstrate that diabetes educator (DE) working with healthcare providers can improve the blood glucose levels, blood pressure, lipid profile and to report that how they enhance the health related of life (HRQol)and Medication adherence

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. Patients equal to or above 18 years of age;
2. Patients with confirmed diagnosis of type 2 diabetes mellitus;
3. Patients receiving both oral hypoglycaemic drugs and/or insulin therapy;
4. Patients who were declared fit by their consultant / physician.

Inclusion criteria

1. Patients equal to or above 18 years of age;
2. Patients with confirmed diagnosis of type 2 diabetes mellitus;
3. Patients receiving both oral hypoglycaemic drugs and/or insulin therapy;
4. Patients who were declared fit by their consultant / physician.

排除标准：

1. Patients with confirmed diagnosis of type-1 diabetes mellitus;
2. Patients with pregnancy;
3. Patients with ESRD, Cirrhosis, Chronic heart disease and other complications;
4. Patients with mental illness or cognitive deficit.

Exclusion criteria：

1. Patients with confirmed diagnosis of type-1 diabetes mellitus;
2. Patients with pregnancy;
3. Patients with ESRD, Cirrhosis, Chronic heart disease and other complications;
4. Patients with mental illness or cognitive deficit.
