研究目的：

1. R2-CHOP联合强化的中枢神经系统预防和阴囊放疗治疗初诊的原发睾丸弥漫大B细胞淋巴瘤的安全性与可行性 2. R2-CHOP联合强化的中枢神经系统预防和阴囊放疗对原发睾丸弥漫大B细胞淋巴瘤的中枢复发和生存的影响  

Objectives of Study：

1. The safety and feasibility of R2-CHOP with intensive central system prophylaxis and scrotal irradiation for newly diagnosed primary diffuse large B cell lymphoma of the testis; 2. The relpase rate and survival of primary diffuse large cell lymphoma with R2-CHOP and intensive central prophylaxis and scrotal irradiation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄在15-75岁
2. 初诊未治疗的原发睾丸弥漫大B细胞淋巴瘤
3. 纳入试验前进行例睾丸切除术且在术后2个月内
4. Ann Arbor分期为IE和IIE期
5. 血细胞计数： 绝对中性粒细胞绝对值大于1.5 x 109/L， 血小板计数大于75 x 109/L
6. 超声心动图提示心脏射血分数不低于45%
7. 无影响化疗的基础疾病
8. 结合胆红素不超过正常上限的2倍
9. 转氨酶不超过正常上限的2倍
10. 肌酐清除率不低于45ml/分
11. HIV和HCV抗体阴性
12. 无活动性慢性乙型肝炎
13. 预期寿命超过6个月
14. ECOG评分小于2分
15. 能充分理解试验并签署知情同意书

Inclusion criteria

1. Aged 15-75 years;
2. Newly diagnosed and untreated primary testicular diffuse large B cell lymphoma;
3. Orchiectomy is performed and within 2 months before study enrollment;
4. Ann Arbor Stage IE and IIE;
5. Hematological counts: ANC > 1.5 x 10^9/L and PLTs count > 75 x 10^9/L;
6. Cardiac ejection fraction >=45% by echocardiography;
7. No underlying disease that may preclude chemotherapy;
8. Conjugated bilirubin <=2 ULN;
9. Transaminases <=2 ULN;
10. Creatinine clearances >=45 ml/min;
11. HIV and HCV negativity;
12. No signs of active chronic hepatitis B;
13. Life expectancy > 6 months;
14. Performance status < 2 according to ECOG scale;
15. Understanding the trial and writing informed consent.

排除标准：

1. 继发性睾丸淋巴瘤
2. 中枢神经系统淋巴瘤浸润
3. 有其他恶性肿瘤史，除非受试者不再患有该疾病≥5年。
4. 移植后淋巴组织增生性疾病
5. 对化疗药物过敏或不能耐受
6. 肝肾功或其他脏器功能不全导致不能耐受化疗
7. 未控制的或严重的心血管疾病，包括入组时近6个月内发生过心肌梗塞或III-IV级心衰
8. 未控制的高血压或糖尿病
9. 参加其他临床试验

Exclusion criteria：

1. Secondary testicular lymphoma;
2. CNS involvement by lymphoma;
3. History of other malignant diseases with the exception of patients with complete remission >=5 years;
4. Post-transplant lymphoproliferative disorders;
5. Hypersensitivity or intolerance to any agent of chemotherapy;
6. Liver or renal insufficiency or other diseases that preclude chemotherapy;
7. Uncontrolled or severe cardiovascular disease including myocardial infarction within 6 months of entry, New York
Heart Association (NYHA) Class III or IV heart failure;
8. Uncontrolled hypertension or diabetes;
9. Have participated in other clinical trial.
