研究目的：

主要目的：探索慢阻肺质量控制管理方式对患者急性加重的影响； 次要目的：探索慢阻肺质量控制管理方式的卫生经济学价值； 探索慢阻肺质量控制管理方式下的患者疾病认知度的改变； 探索慢阻肺质量控制管理方式下的患者教育满意度的影响。  

Objectives of Study：

Main objective: To explore the effect of quality control management on exacerbations of chronic obstructive pulmonary (COPD). Secondary objective. 1.To explore the health economic value of quality control management of COPD. 2.To explore the changes of patients' disease awareness with the quality control management of COPD. 3.To explore the influence of the quality control management of COPD on patients' educational satisfaction.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.有COPD病史且1年内有肺功能诊断证明存在不可逆性气流受限（支气管舒张剂后FEV1/FVC小于70%）；
2.患者年龄≥ 40岁；
3.自愿参与本次临床研究并签署知情同意书。

Inclusion criteria

1. History of COPD and FEV1/FVC less than 70% after bronchodilator within 1 year.
2. Aged >= 40 years.
3. Volunteer to participate in this study and sign the informed consent form.

排除标准：

1. 患有COPD以外的其他严重肺部疾病（严重支气管扩张症、肺癌、囊性肺纤维化病史、已知的活动性肺结核、筛选前6个月内需要支气管内介入治疗的重度肺气肿）、危及生命的心律失常、先天性心脏病的患者；
2. 接受过肺部手术的患者；
3. 孕妇或哺乳期妇女；
4. 有语言障碍、认知障碍及意识障碍的患者（不能签署知情、问卷等表格）；
5. 无法通过电话随访的患者；
6. 医师根据患者实际情况判断不适合参与本次临床研究。

Exclusion criteria：

1. Patients with severe lung diseases other than COPD (severe bronchiectasis, lung cancer, history of cystic pulmonary fibrosis, known active pulmonary tuberculosis, severe emphysema requiring endobronchial interventional therapy within 6 months before screening), life-threatening arrhythmias, congenital heart disease.
2. Patients who have undergone lung surgery.
3. Pregnant or lactating women.
4. Patients with language disorder, cognitive impairment and consciousness disorder (can not sign informed, questionnaire and other forms).
5. Patients who cannot be followed up by phone.
6. Doctors judge that it is not suitable to participate in this clinical study according to the actual situation of patients.
