研究目的：

探索血小板IIbIIIa受体拮抗剂：替罗非班对急性心肌梗死患者行急诊PCI术时对血小板活化度和内皮功能的影响。  

Objectives of Study：

Effects of glycoprotein (GP) IIb/IIIa inhibitor tirofiban on platelet activation and endothelial function in patients with ST-elevation myocardial infarction undergoing primary percutaneous coronary intervention.

药物成份或治疗方案详述：

替罗非班 

Description for medicine or protocol of treatment in detail：

tirofiban 

纳入标准：

1、胸痛持续小于12小时。2.心电图至少2个连续导联ST段抬高。3 血清肌钙蛋白和心肌酶升高。

Inclusion criteria

1) presence of angina for < 12 hours prior to hospital admission; (2) elevation of ST-segment in at least 2 consecutive ECG leads; and (3) elevation of serum cardiac troponin T or myocardial enzymes.

排除标准：

血流动力学不稳定：在没有应用任何升压药前提下，血压低于90/60mmHg，出现心源性休克。AMI机械并发症：二尖瓣脱垂、室间隔穿孔。血液系统疾病包括：血常规：Hb<10g/l，PLT<100X109/L，因为贫血和血小板减低会增加应用抗血小板药物和抗凝药物出血风险。难以控制的高血压没有排除，如果在术中发现血压异常升高，SBP>180mmHg，会在术中应用静脉降血压药物控制血压。近期手术患者会排除。

Exclusion criteria：

(1) manifestation of cardiac shock or a blood pressure (BP) < 90/60 mmHg; (2) evidence of severe complications of MI, including mitral valve prolapse and perforation of the ventricular septum; (3) involvement of multiple coronary arteries in patients who were suitable for a coronary artery bypass graft; (4) presence of contraindications for use of anticoagulant therapy, such as severe and uncontrolled hypertension (systolic BP > 180 mmHg despite intravenous use of antihypertensives), major surgery or trauma within 6 weeks, hemorrhagic stroke within 6 months, major gastrointestinal bleeding within 6 months; (5) serum hemoglobin < 100 g/L or platelets < 100 × 109/L; (6) presence of severe renal or hepatic dysfunction.
