研究目的：

1、在内蒙古地区建立高血压信息化管理平台。 2、探讨高血压远程管理是否可以提高内蒙古地区高血压患者的治疗达标率。 3、远程管理对提高患者用药依从性有无作用 4、远程管理是否在改善生活方式及控制可控的心血管危险因素方面更具优势。  

Objectives of Study：

1. Establish an information management platform for hypertension in Inner Mongolia. 2. To explore whether remote hypertension management can improve the treatment compliance rate of hypertensive patients in Inner Mongolia. 3. Does remote management have any effect on improving patient compliance with medication? 4. Does remote management have more advantages in improving lifestyle and controlling controllable cardiovascular risk factors

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）原发性高血压患者: 包括
1）初诊高血压患者，门诊随访3次平均收缩压≥140 mmHg；
2）正在接受降压治疗的高血压患者。
（2）长期居住于内蒙古地区，年龄18～80岁（18岁≤年龄<80岁）；
（3）签署知情同意书。

Inclusion criteria

1. Patients with essential hypertension:
1) For newly diagnosed hypertensive patients, the average systolic blood pressure >=140 mmHg during 3 outpatient follow-up;
2) Hypertensive patients undergoing antihypertensive therapy.
2. Long-term living in Inner Mongolia, aged 18 to 80 years old;
3. Sign the informed consent form.

排除标准：

（1）收缩压SBP≥190 mmHg，或者舒张压<60 mmHg；
（2）近6个月内因急性心肌梗死或不稳定性心绞痛住院治疗；
（3）在未来6个月内计划实施血运重建手术者（PCI或CABG）；
（4）有持续性房颤或心律失常影响电子血压计测量；
（5）筛查时伴有NYHAIII、IV级的心力衰竭；
（6）严重肝或肾脏疾病（ALT ≥正常上限3 倍；透析治疗的患者或肾小球滤过率估算值[eGFR]<30ml/min，或血肌酐>2.5 mg/dl [>221 mol/L]）；
（7）恶性肿瘤病史；
（8）有认知功能障碍者或生活无法自理者；
（9）正在参加其他临床试验者。

Exclusion criteria：

(1) SBP>=190 mmHg for systolic blood pressure, or <60 mmHg for diastolic blood pressure;
(2) Hospitalization for acute myocardial infarction or unstable angina pectoris within the past 6 months;
(3) Those who plan to perform revascularization surgery (PCI or CABG) within the next 6 months;
(4) Persistent atrial fibrillation or arrhythmia affects the measurement of the electronic sphygmomanometer;
(5) Heart failure of NYHAIII and IV during screening;
(6) Severe liver or kidney disease (ALT >=3 ULN; dialysis patients or estimated glomerular filtration rate [eGFR]<30ml/min, or blood creatinine>2.5 mg/dl [>221 mol/ L]);
(7) History of malignant tumor;
(8) Those with cognitive dysfunction or those unable to take care of themselves;
(9) Those who are participating in other clinical trials.
