研究目的：

探索新辅助放化疗联合特瑞普利单抗序贯手术治疗胸段食管鳞癌的安全性与有效性。  

Objectives of Study：

Explore the safety and efficacy of neoadjuvant chemoradiation combined with Toripalimab in sequential surgery for thoracic esophageal squamous cell carcinoma.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.18-70岁, PS评分 0-1 ；
2.食管外科专家认为可切除或潜在可切除的病理证实的胸段食管鳞癌；
3.术前首选PET-CT检查，次选胸腹部增强CT、骨扫描检查，根据第八版AJCC/UICC分期为III-IVA期，具体：T2N+（106rec组或腹部1、2、3、7、8、9组淋巴结转移）；T3-4a Any N；M0患者；
4.预计生存期超过半年；
5.WBC≥4.0×10^9/l, ANC≥1.5×10^9/l，血小板≥100.0×10^9/l, Hb≥90g/l，肝肾功能正常；
6.一般情况或内科合并症能够耐受放化疗及手术；
7.理解并签署本研究知情同意书。

Inclusion criteria

1. For subjects aged 18-70, PS score was 0-1;
2. Patients with pathologically proved thoracic esophageal squamous cell carcinoma who are considered resectable or potentially resectable by esophageal surgeons;
3. The first choice of preoperative PET-CT examination was chest and abdomen enhanced CT and bone scan. According to the eighth edition AJCC / UICC staging, it was stage III-IVA, specifically: T2N + (106rec group or abdominal 1, 2, 3, 7, 8, 9 groups of lymph node metastasis); T3-4a Any N M0 patients;
4. Patients whose expected survival time is more than half a year;
5. WBC >= 4.0 x 10^9/l, ANC >= 1.5 x 10^9 / L, platelet >= 100.0 x 10^9 / L, Hb >= 90 g / L, normal liver and kidney function;
6. Patients with general conditions or medical complications who can tolerate radiotherapy, chemotherapy and surgery;
7. Patients who understand and sign the informed consent form of this study.

排除标准：

1.锁骨上（104组）淋巴结转移，106rec转移淋巴结超过胸廓入口水平延伸至101组，食管外科专家认为无法清扫；
2.既往恶性肿瘤病史，既往接受放化疗。

Exclusion criteria：

1. For the patients with supraclavicular lymph node metastasis (group 104), 106rec lymph nodes extended beyond the level of thoracic entrance to group 101, which was considered impossible by esophageal surgery experts;
2. Patients with previous history of malignant tumor, who have received radiotherapy and chemotherapy in the past.
