研究目的：

观察高碳酸血症对非气管插管全麻胸腔镜手术患者术后认知功能的影响。通过运用局部脑氧饱和度( rScO2)监测手段指导临床PETCO2的调控，以增加脑氧供，降低神经系统损伤的发生率,为临床工作提供参考。  

Objectives of Study：

To observe the effect of hypercapnia on cognitive function in patients undergoing non-intubated spontaneous-ventilation video-assisted thoracoscopic surgery.Through the use of local cerebral oxygen saturation (rsCO2) monitoring method to guide the clinical regulation of PETCO2, in order to increase cerebral oxygen supply and reduce the incidence of nervous system injury,to provide reference for clinical work.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①行单一肺楔形切除；
②手术时间<2小时且术中出血量<300 ml;
③无广泛胸腔粘连;
④ASAⅠ-Ⅱ级;
⑤无其它椎旁神经阻滞及肋间神经阻滞相关禁忌证症。

Inclusion criteria

1.Patient who have undergone simple surgeries such as pulmonary wedge resection
2.The operation duration is no more than 2 hours and peri-operative<300 ml
3.There were no extensive pleural adhesion
4.American Society of Anesthesiologists Standard(ASA) grade of Ⅰ-Ⅱ
5.There were no contraindication related to paravertebral nerve and intercostal nerve block

排除标准：

①拒绝手术或麻醉方案，既往胸腔镜手术史；
②中-重度慢性阻塞性肺疾病 （COPD），术前低氧血症(PaO2<60 mmHg)或高碳酸血症 (PaCO2>50mmHg)，严重急性肺部感染
③术前检查提示心律失常，心功能不全；
④局麻药过敏，凝血功能障碍
⑤体重指数（BMI）>28 kg/m2；
⑥广泛胸膜粘连或合并复合肺叶和或肺段切除；
⑦中转插双腔气管导管或中转开胸；
⑧合并神经精神疾病或其他重要器官功能不全。

Exclusion criteria：

1.refuses the surgery and anesthesia protocols,has a history of surgery in the ipsilateral thoracic cavit
2.moderate-severe COPD,hypoxemia (PaO2<60 mmHg)or hypercapnia (PaCO2>50mmHg) preoperatively,associated severe acute pulmonary infection
3.There is no indications of arrhythmia,cardiac insufficiency
4.allergy to local anesthesia,coagulopathy
5.body mass index (BMI)>28kg/m2
6.extensive pleural adhesion,lobectomies,segment resections
7.intubated video-assisted thoracoscopic surgery or thoracotomy
8.associated neuropsychological diseases or other vital organ dysfunction
