研究目的：

在一年的随访时间内，研究NobelParallel? CC种植体联合上颌窦侧壁开窗提升术在上颌后牙区骨量不足患者中的临床应用效果，作为PCC组，并同时研究其他种植体联合上颌窦侧壁开窗提升术在上颌后牙区骨量不足患者中的临床效果，作为对照组。同时设立PCC对照组及其他对照组，分别研究NobelParallel? CC种植体在上颌后牙区骨量充足情况下的疗效，及上颌后牙区骨量严重不足时联合上颌窦侧壁提升术的临床表现。检测各组的种植体保存率、种植体成功率、种植体稳定度、边缘骨吸收、种植体周情况、植体周上颌窦内骨增量、上颌窦并发症发生率、患者满意度等，评估上颌窦侧壁开窗提升术的初期稳定性及术后并发症的发生率，同时与基线值相比探讨其骨增量效率；此外，通过统计各组间差异，分析NobelParallel? CC种植系统应用于口腔种植领域，尤其是联合上颌窦侧壁开窗提升术的临床疗效。  

Objectives of Study：

Within the one-year period of following up, the clinical effect of Nobel parallel CC implant in combination with maxillary sinus lateral wall elevation in patients with maxillary posterior area bone deficiency is observed, as PCC group. And Nobel parallel CC implants are replaced by other systems as control group. PCC control group and other control groups were simultaneously set up to analyze the clinical efficacy of Noble parallel CC implant in the presence of sufficient bone mass in the maxillary posterior area. The implant survival rate, implant success rate, implant primary stability, marginal bone resorption, peri-implant bone gain, incidence of complications in maxillary sinus and patient satisfaction were detected in each group to evaluate the initial stability and incidence of postoperative complications of lateral elevation of the maxillary sinus, and to explore the efficiency of bone gain compared with the baseline value.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、自愿入组，知情同意；
2、年满18周岁；
3、上颌后牙区（磨牙和前磨牙）单颗牙缺失，对颌牙及双侧邻牙为天然牙或已行修复；
4、对照组缺牙区剩余骨高度不少于10mm；PCC组及PCC对照组缺牙区剩余骨高度不高于6mm；
5、PCC组患者在侧壁开窗上颌窦底提升术时联用Nobel PCC种植体，对照组患者在侧壁开窗上颌窦底提升术时联用其他品牌植体；
6、缺牙区牙槽嵴宽度不少于6mm（能够植入直径4.5mm种植体）。

Inclusion criteria

1informed consent was obtained
2patients aged >=18 years
3loss of single tooth in posterior maxillae (molar or premolar), in need for one implant
4、there is vertical resorption at tooth loss site, and the vertical height of residual ridge is over 10 mm for the control group, while the vertical height of residual ridge is less than 6mm for atrophic group
5、patients receive treatment involving lateral elevation of the maxillary sinus in combination with the placement of NobelParallel CC implants in PCC group; while in control group, patients receive lateral elevation of the sinus as well as simultaneous implantation of implants of other brands.
6、horizontal width of the residual bone at the edentulous site is over 6mm

排除标准：

1、不能耐受种植手术者（因心脏病、高血压等系统性疾病等）；
2、一年内头颈部放化疗史；
3、口腔卫生条件差（全口菌斑指数的中位数≥2）；
4、控制不佳的糖尿病（糖化血红蛋白水平＞8.0％或空腹血糖＞150mg/dL）；
5、怀孕或妊娠期；
6、有精神疾病或幻想症者；
7、HIV感染患者
8、每日吸烟至少20支
9、种植区有良性或恶性肿瘤；
10、有静脉使用双膦酸盐史；
11、局部炎症，包括未控制的牙周炎
12、拔牙未满3月者
13、重度磨牙或紧咬牙习惯
14、骨质缺陷性疾病如重度骨质疏松

Exclusion criteria：

1general contraindications to implant surgery
2subjected to irradiation in the head and neck area within two years prior to treatment
3、poor oral hygiene conditions (median of full mouth plaque index >=2)
4、uncontrolled diabetes (HbA1c>8.0% or FBG>150mg/dl)
5、pregnant or nursing
6、psychiatric problems
7、HIV infected patients
8、heavy smoking(>20 cigarettes/day)
9、benign or malignant tumors in the area intended for implant placement
10、treated or under treatment with intravenous amino-bisphosphonates
11、acute or chronic local inflammation including untreated periodontitis
12、extraction sites with less than 3 months of healing.
13、severe bruxism or clenching habits
14、bone defect diseases such as osteoporosis

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

通过填写CRF表，进行数据采集。然后对病例报告表中数据进行核查，发现疑问向研究者询问，根据研究者的回答进行数据修改、确认；将CRF的数据输入计算机，建立数据库并进行二次检错，数据锁定和储存后，由统计分析人员进行分析。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Researchers collect the data by CRF,then check the data in CRF, ask the researchers if there are any questions, and modify and confirm the data according to the answers given by the researchers; after the data is entered into the computer, establish a database and double error detection of data. After the data is locked and stored， it is analyzed by statistical analysts.
